丹参川芎嗪注射液防治冠状动脉介入诊疗术后对比剂肾病的有效性研究

摘　　要：目的:探讨丹参川芎嗪注射液对预防和治疗冠状动脉介入诊疗术患者术后发生对比剂肾病(CIN)的相关作用及机制。方法:选择稳定型冠心病患者158例,随机分为对照组80例和丹参川芎嗪治疗组78例,两组同时行常规水化治疗,治疗组额外注射丹参川芎嗪制剂,检测两组术前与术后36 h、72 h后C-反应蛋白(CRP)、尿素氮(BUN)、血肌酐(Scr)、肌酐清除率(Ccr)和CIN发病率变化。结果:术后36 h,治疗组Ccr下降程度与BUN、CRP、SCr增加程度较对照组减少(P>0.05);72 h后,治疗组Ccr下降程度与Scr、CRP、BUN增加程度与对照组Ccr下降及CRP、BUN、Scr增加程度相比减少明显(P<0.05);两组CIN发生率对照组(12.8%)高于治疗组(3.7%,P<0.05)。结论:丹参川芎嗪注射液可明显改善冠脉造影术后肾功能,降低对比剂肾病的发生率。Objective：To investigate the preventive effect ofDanshen Chuanxiongqin injection on contrast-induced nephropathy （CIN）in patients undergoing elective percutaneous coronary intervention（PCI）. Methods： 158 patients were randomly divided into control group and treatment group. Routine hydration treatment was performed in the two groups,while the treatment group was also treated with Danshen Chuanxiong injection. BUN, Scr and Ccr were detected before and 36 hours, 72 hours after coronary angiography in all patients. Meanwhile,the rate of mortality was also observed. Results：At 36 h after surgery, the decreased degree of Ccr and BUN,CRP and the increased degree of Scr of treatment group were less than of control group. At 72 h after surgery,the decreased degree of Ccr and the increased degree of Scr,CRP and BUN of treatment group was less than that of control group （P〈0.05）. The prevalence of CIN of eontrol group was higher than that of treatment group（P〈0.05）. Conclusion ：Application of Danshen Chuanxiongqin injection before coronary angiography couhl obviously improve the ranal functions of the patients, and significantly decrease the incidence of contrast-induced nephropathy.
