维持性血液透析患者贫血相关因素分析

摘　　要：目的通过了解维持性血液透析患者贫血状态,分析贫血相关因素,并根据数据调整治疗方案,改善患者生存质量。方法收集2016年9月在石河子大学医学院第一附属医院肾病科维持性血液透析超过3个月的130例患者的临床和实验室资料,分析患者贫血的相关因素。结果 130例患者,平均透析龄P_25-P_75(20,65)月,P_50(63月)。其中血红蛋白(Hb)未达到110 g/L者79例。Hb未达标者与Hb达标患者相比,血钙、血磷水平均无明显差异(P>0.05),但血浆白蛋白、血清铁蛋白、甲状旁腺素与贫血相关(P<0.05)。多因素分析显示,男性有利于贫血的纠正,而血清铁蛋白增高、甲状旁腺素增高患者贫血更难纠正。结论我院维持性血液透析患者贫血达标率偏低,在保证铁储备情况下,加强蛋白摄入、维持良好的营养状态,控制继发性甲状旁腺功能亢进,对控制患者的贫血是有意义的。Objective To investigate the prevalence of anemia and the factors relating to anemia for the management of anemia and improvement of life quality in maintenance hemodialysis patients. Methods Kidney disease maintenance hemodialysis more than three months of 130 eases of patients with clinical and laboratory data were collected, analyzed the related factors of anemia in patients. Results 130 patients, with an average age of dialysis P25-P75（20, 65） month, P50（63 month）. The hemoglobin （Hb） have not reached 79 cases of 110 g/L, compared with standard Hb patients, blood calcium and blood phosphorus levels had no obvious difference（P〉0.05 ）, but serum albumin, serum ferritin, parathyroid hormone associated with anemia （P〈0.05）. Multiple factors analysis showed that men for correcting anemia, and increased serum ferritin, higher parathyroid hormone in patients with anemia, more difficult to correct. Conclusion Anemia in patients with maintenance hemodialysis in our success rate is low, in the case of guarantee iron reserves, strengthen the protein intake, to maintain good nutritional status, control of secondary hyperparathyroidism, is meaningful to control in patients with anemia.
