摘 要:目的:探讨宫颈病变中人类染色体端粒酶(hTERC)基因的异常扩增情况及其与人乳头瘤病毒感染在宫颈病变中的关系。方法:选取2014年2月—2015年10月在我院就诊的宫颈上皮内瘤变及宫颈鳞癌患者66例(观察组),其中宫颈上皮内瘤CINⅠ13例,CINⅡ18例,CINⅢ22例,宫颈癌13例,同时选取宫颈组织正常及宫颈炎症20例作为对照组,检测各组标本hTERC基因的异常扩增以及人乳头瘤病毒感染情况。结果:86例宫颈样本中,hTERC基因异常扩增阳性率为50.00%,其中对照组、CINⅠ组、CINⅡ组、CINⅢ组、宫颈癌组的hTERC基因异常扩增阳性率分别为0.00%、30.77%、61.11%、77.27%和84.62%,提示随着宫颈病变程度升高,hTERC基因异常扩增阳性率有逐渐升高的趋势(X2=33.349,P<0.05);正常或炎症、CINⅠ组、CINⅡ组、CINⅢ组、宫颈癌组的HPV感染率分别为55.00%、69.23%、77.78%、81.82%和92.31%,提示随着宫颈病变程度升高,HPV感染阳性率有逐渐升高的趋势(X^2=6.749,P<0.05);Spearson秩相关分析结果显示hTERC基因异常扩增与HPV感染呈相关关系(rs=0.529,P<0.05)。结论:hTERC基因的异常扩增与宫颈病变程度和HPV感染有关,其在临床诊治中有重要的意义。Objective:The aim of this article is to investigate the abnormal amplification of human telosome telomerase(hTERC)gene in cervical lesions and its relationship with human papillomavirus infection in cervical lesions.Methods:We collected 66 patients with cervical intraepithelial neoplasia and cervical squamous cell carcinoma(obs grervationoup)in our hospital for treatment from February in 2014 to October in 2015,among which there are 13 cases of cervical intraepithelial neoplasia(CIN),18 cases of CINⅡ,CINⅢin 22 cases,13cases of cervical cancer.At the same time,we selected 20 casesof normal cervical tissue and cervicitis as the control group to detect each group's specimens abnormal amplification of hTERC gene and HPV infection.Results:In the 86 cases of cervical samples,abnormal amplification positive rate of hTERC gene was 50.00%,while the positive rate of abnormal hTERC gene amplification in CINⅠ,CINⅡ,CINⅢ and Cervical cancer group respectivelywere 0.00%、30.77%、61.11%、77.27% and 84.62%,pointing out that,with the raising of the degree of cervical lesions.hTERC abnormal rate of abnormal amplification gradually trends to increase(X^2=33.349,P〈0.05);The infection rate of HPV in normal or inflammatory,CINⅠ,CINⅡ,CINⅢand cervical cancer group respectivelywere 55.00%、69.23%、77.78%、81.82% and 92.31%,showing that with the increased degree of cervical lesions,the positive rate of HPV infection also increases gradually(X^2=6.749,P〈0.05);Spearson rank correlation analysis showed that the abnormal amplification of hTERC gene has associated with HPV infection(rs=0.529,P〈0.05).Conclusion:The abnorml amplification of hTERC gene is related to the degree of cervical lesions and HPV infection,which is of great significance in clinical diagnosis and treatment.
