摘  要:目的:探讨以类胰蛋白酶抗体与C3a受体抗体检测肾组织肥大细胞(MC)浸润的可行性。方法:以正常肾组织为对照(n=6),选择乙型肝炎病毒相关性IgA肾病(HBV-IgAN,n=6)和原发性IgA肾病(pIgAN,n=6)为研究对象,采用免疫组化方法分别以类胰蛋白酶抗体、C3a受体抗体检测肾组织肥大细胞浸润情况。结果:C3aR及类胰蛋白酶阳性肥大细胞呈圆形或椭圆形,大小不等,胞浆着色,呈棕褐色颗粒,分布基本一致,但未完全重叠。正常肾组织MC在肾间质偶有浸润,肾小球内未见浸润。与正常对照组相比,HBV-IgAN与pIgAN肾间质肥大细胞浸润显著增加,主要分布于肾小管周围间质区域,以纤维化区域多见。C3aR阳性的肥大细胞数目与类胰蛋白酶阳性肥大细胞数目显著正相关(r=0.975;P<0.05)。但两种疾病之间肥大细胞数量差异无统计学意义。结论:类胰蛋白酶抗体及C3aR抗体两种免疫组化方法均适用于肾组织肥大细胞的检测。肾组织肥大细胞浸润在HBV-IgAN和pIgAN进展中的作用有待研究。Objective: To discuss the feasibility of using Tryptase antibodies and C3 a receptor( C3aR) antibodies to measure mast cell( MC) infiltration in kidney tissues. Methods: Using normal kidney tissues as the control group( n = 6),hepatitis B virus associated IgA nephropathy( HBV-IgAN,n = 6) and primary IgA nephropathy( pIGAN,n = 6) as experimental groups,apply immunohistochemistry with Tryptase antibodies and C3 aR antibodies respectively to measure MC infiltration in renal tissues. Results:C3aR and Tryptase positive mast cells are round or oval in shape,varying by size,coloured in cytoplasm. They appear to be brown granule with similar distribution for both types of antibodies but do not overlap completely. In normal renal tissues,MCs infiltrate sometimes in renal interstitial but never in glomeruli. In contrast,MC infiltration in renal interstitial significantly increased in HBV-IgAN and pIgAN renal tissues,mostly distributing around interstitial areas near renal tubules,more commonly in fibrosis areas. The number of C3 aR positive MCs correlates positively with the number of Tryptase positive MCs( r = 0. 975; P 0. 05). There is,however,no significant difference in the number of MCs between the two diseases. Conclusion: Immunohistochemistry using Tryptase antibodies or C3 aR antibodies are both effective in detecting MCs in renal tissues,while further research is needed to understand the role of MC infiltration in the progression of HBV-IgAN and pIgAN.
