经胸超声和造影法在国产封堵器治疗先心病中的应用效果

摘　　要：目的:探讨采用国产封堵器介入治疗先心病的临床应用效果。方法:选取2014到2016年我院收治的实施国主封堵器介入治疗的先心病患者102例,其中22例动脉导管未闭患者,45例房间隔缺损患者,余下均为室间隔患者。应用经胸超声心动图和造影技术进行术前诊断,术中观察及术后随访。结果:102例患者中有100例患者成功安装好封堵器,成功率为98.2%,术后24小时虽有4例患者有残余分流现象,但经一个月的治疗后分流消失,经一年随访,所有患者均未出现异常现象。结论采用国产封堵器介入治疗简单先心病具有安全可靠、手术成功率高、并发症少、操作简单、经济实惠的特点,可在临床大力推广,其中经胸超声在手术前诊断中起重要作用,并可监测ASD手术中置入封堵器和测定缺损最大直径;造影法有利于PDA介入手术和VSD手术前缺损直径的测量,放置封堵器后在造影结合经胸超声心动图观察效果较好,二者在先心病手术中均起着关键作用。Objective to investigae the clinical effect of using domestic occluder device in the treatment of congenital heart disease. January 2014 to 2016 January in our hospital during the period of implementation of domestic occluder interventional treatment of patients with congenital heart disease,of which 22 cases ofpatent ductus aarteriosus patients,45 patients with atrial septal defect patients,the rest are for ventricular septal defect,application in ultrasonic echocardiography and angiography technique for preoperatie diagnosis and introperative observation andpostoperative follow-up. Results 92 patients with successful installation of good domestic occluder in 90 cases ofpatients,the success tate was 98. 2%,posoperative 24 hours although 4 cases of patients with residual shunt phenomenon but after a month of treatment shunt all disappeard,after 1 year of follow-up,all patients did not appear abnormal phenomenon. Conclusion the domestic made occluder interventional treatment of congenital heart disease is safe and reliable,successful operation rate is high,less complications,simple,affordable,to popularize in clinical,which transthoracic echocardiography in the preoperative diagnosis of play an important role, and monitoring ASD surgical placement of occluder and the determination of the maximum diameter of the defect; cotrast the rules in favor of PDA and VSD surgery in maximum diameter measurements,in order to facilitate apptrpriate occluder selection,both in the domestic operation plays a key role.
