彩色多普勒超声引导下经皮经肝胆管穿刺置管引流术50例临床治疗体会

摘　　要：目的:分析总结梗阻性黄疸患者在彩色多普勒超声引导下行经皮经肝胆管穿刺置管引流术的治疗体会。方法:选取50例2015年1月至2017年1月我院收治的梗阻性黄疸患者,在彩色多普勒超声引导下对其实施经皮经肝胆管穿刺置管引流术治疗,观察患者的临床治疗效果。结果:患者治疗7d、14d后的胆红素水平与治疗前比较均显著较低,差异明显(P<0.05)。本组50例患者均手术成功,一次穿刺成功率为96.0%,术后平均胆汁引流量为(456.3±22.3)m L,术后黄疸、全身瘙痒等症状均消退,术后并发症发生率为6.0%。结论:在彩色多普勒超声引导下对阻塞性黄疸患者实施经皮经肝胆管穿刺置管引流术,有助于缓解患者临床症状,降低胆红素水平,提高治疗效果。Objective： to analyze and summarize the experience of treatment of obstructive jaundice by color Doppler ultrasound guided percutaneous transhepatic bile duct drainage. Methods： 50 cases of patients with obstructive jaundice from January 2015 to January 2017 in our hospital,under the guidance of color Doppler ultrasound in percutaneous transhepatic biliary drainage treatment,clinical curative effect observation of patients. Results： After treatment with 7d and 14 d,the level of bilirubin was significantly lower than that before treatment,the difference was significant（ P 〈 0. 05）. This group of 50 patients were operated successfully,the success rate of puncture was 96%,the average postoperative biliary drainage for（ 456. 3＋ 22. 3） m L,postoperative jaundice,pruritus and other symptoms have subsided,postoperative complication rate was 6%.Conclusion： color Doppler ultrasound guided percutaneous transhepatic bile duct drainage in patients with obstructive jaundice,help to alleviate the clinical symptoms,reduce the level of bilirubin,improve the therapeutic effect.
