血浆D-二聚体、C反应蛋白水平及白细胞计数在评价社区获得性肺炎严重程度及预后中的价值

摘　　要：目的分析在社区获得性肺炎(CAP)患者严重程度与预后评价中应用白细胞计数(WBC)、血浆D-二聚体与C反应蛋白(CRP)水平的价值。方法 98例社区获得性肺炎患者,按照患者肺炎严重度指数(PSI)评分标准分为低死亡危险组(70例)和高死亡危险组(28例);按照肺炎病情严重程度分为重症肺炎组(30例)和非重症肺炎组(68例)。比较低死亡危险组和高死亡危险组、重症肺炎组和非重症肺炎组患者WBC、血浆D-二聚体与CRP水平。结果低死亡危险组患者WBC、血浆D-二聚体与CRP水平均低于高死亡危险组患者,差异具有统计学意义(P<0.05)。重症肺炎组患者WBC、CRP与血浆D-二聚体水平分别为(9.27±0.94)×109/L、(133.75±13.58)mg/L、(4.61±0.76)mg/L,均高于非重症肺炎组患者的(7.75±0.85)×109/L、(64.82±5.82)mg/L、(0.81±0.49)mg/L,差异具有统计学意义(P<0.05)。结论 WBC、血浆D-二聚体与CRP水平和社区获得性肺炎患者病情密切相关。Objective To analyze the value of white blood count （WBC）, plasma D-dimer and C-reactive protein （CRP） in assessing the severity and prognosis of community-acquired pneumonia （CAP） patients. Methods A total of 98 community-acquired pneumonia patients were divided by pneumonia severity index （PSI） score criteria in to low death risk group （70 cases） and high death risk group （28 cases）, and by severity of pneumonia into severe pneumonia group （30 cases） and non severe pneumonia group （68 cases）. Comparison were made on WBC, plasma D-dimer and CRP level of low death risk group, high death risk group, severe pneumonia group and non severe pneumonia group. Results The low death risk group had lower WBC, plasma D-dimer and CRP level than high death risk group, and their difference had statistical significance （P〈0.05）. The severe pneumonia group had higher WBC, CRP and plasma D-dimer level respectively as （9.27 ± 0.94） × 10^9/L, （133.75 ±13.58） mg/L and （4.61 ± 0.76） mg/L than （7.75 ± 0.85） × 10^9/L, （64.82 ± 5.82） mg/L and （0.81 ± 0.49） mg/L in non severe pneumonia group, and their difference had statistical significance （/9〈0.05）. Conclusion WBC, plasma D-dimer and CRP level was closely related to the condition of community acquired pneumonia patients.
