瑞芬太尼复合丙泊酚静脉麻醉在腹腔镜下胆囊切除中的麻醉效果及对认知功能的影响研究

摘　　要：目的分析瑞芬太尼复合丙泊酚静脉麻醉在腹腔镜下胆囊切除中的麻醉效果及对认知功能的影响。方法 70例腹腔镜胆囊切除患者,按照随机分配方式将其分为研究组与对照组,每组35例。对照组给予异氟醚麻醉治疗,研究组给予瑞芬太尼复合丙泊酚静脉麻醉治疗。对比两组患者的麻醉效果。结果插管前(T0)、麻醉5 min(T1)时,两组患者的心率(HR)、收缩压(SBP)、舒张压(DBP)比较,差异均无统计学意义(P>0.05)。麻醉30 min(T2)、术后5 min(T3)、术后30 min(T4)时,研究组的HR、SBP、DBP均优于对照组,差异均具有统计学意义(P<0.05)。研究组患者的呼吸恢复时间(4.5±1.0)min、苏醒时间(6.3±2.3)min、定力恢复时间(12.3±5.0)min均短于对照组(7.8±1.1)、(10.4±3.1)、(27.4±8.4)min,差异均具有统计学意义(P<0.05)。结论针对腹腔镜胆囊切除患者,实施瑞芬太尼复合丙泊酚静脉麻醉,具有起效快的特点,起着良好镇痛作用,能够快速消除疼痛,值得临床推广运用。Objective To analyze the anesthesia effect of remifentanil combined with propofol intravenous anesthesia in laparoscopic cholecystectomy and its influence on cognitive function. Methods A total of 70 patients with laparoscopic cholecystectomy were randomly divided into research group and control group, with 35 cases in each group. The control group received isoflurane anesthesia for treatment, and the research group received remifentanil combined with propofol intravenous anesthesia for treatment. Anesthetic effect was compared in two groups. Results Both groups had no statistically significant difference in heart rate （HR）, systolic blood pressure （SBP）, and diastolic blood pressure （DBP） before intubation （T0）, anesthesia for 5 min （T1）（P〉0.05）. The research group had better HR, SBP and DBP than the control group at anesthesia for 30 rain （T2）, 5 min after surgery （T3）, 30 min after surgery （T4） （P〈0.05）. The research group had shorter respiratory recovery time as （4.5 ± 1.0） min, wake-up time as （6.3 ± 2.3）min and concentration recovery time as （12.3± 5.0） min than （7.8 ± 1.1）, （10.4 ± 3.1） and （27.4 ±8.4） min the control group, and their difference had statistical significance （P〈0.05）. Conclusion For patients with laparoscopic cholecystectomy, combination of remifentanil and propofol intravenous anesthesia have the characteristics of quick onset, good analgesic effect, rapid elimination of pain, and it is worthy of clinical promotion and application.
