益气固脱、化瘀解毒法治疗脓毒症的临床研究

摘　　要：目的观察血必净联合参麦注射液对脓毒症患者抢救治疗效果及对炎症因子的影响。方法符合诊断标准的脓毒症患者60例,随机分配到2组(对照组30例,观察组30例),2组患者均采用西医基础治疗,观察组在西医基础治疗上加用血必净注射液和参麦注射液,主要评价患者14天病死率,计算APACHEⅡ评分及氧合指数,测定血乳酸(Lac)、血清IL-6、白细胞及降钙素原(PCT)水平。结果血必净联合参麦注射液可明显降低脓毒症患者APACHEⅡ评分及血Lac水平(P<0.05),明显降低血IL-6及PCT水平(P<0.05),还可降低脓毒症患者14天死亡率及白细胞水平,升高氧合指数。结论血必净联合参麦注射液可对体内炎性因子进行调节,增加脓毒症的抢救成功率,本研究可为我中医院制定脓毒症休克诊疗方案及临床路径提供依据。Objective To observe the effect of Xuebijing injection combined with Shenmai injection in the treatment of patients with sepsis and its effect on inflammatory factors. Methods According to the diagnostic criteria, 60 cases of patients with sepsis were randomly assigned to the control group （n = 30） and the observation group （n = 30）. Two groups of patients were treated with Western medicine. On the basis of Western medicine treatment, the observation group was also given Xuebijing injection and Shenmai injection. The main evaluation of 14 day mortality was carried out. APACHE Ⅱ score and oxygenation index were calculated. Blood lactic acid （Lac）, serum IL-6, white blood cell and procalcitonin （PCT） level were detected. Results Xuebijing combined with Shenmai injection can significantly reduce the level of Lac score and blood of patients with sepsis APACHE II （P 〈 0.05）, decrease blood IL-6 and PCT levels （P 〈 0.05）, and also can decrease 14d mortality and reduce white blood cell level, and increase oxygenation index. Conclusion Xuebijing combined with Shenmai injection can regulate vivo inflammatory factors, increase the rescue rate of septic, and the success rate of this study can provide the basis for the formulation of our hospital of traditional Chinese Medicine septic shock treatment and clinical pathway.
