摘 要:目的研究2型糖尿病(T2DM)并发骨质疏松与中性粒细胞/淋巴细胞的比值(NLR)、红细胞分布宽度(RDW),血小板平均体积(MPV)的关系。方法选择从2015年1月到2017年1月在首都医科大学附属北京友谊医院医院进行治疗的T2DM患者314例纳入本次研究。根据双能X线吸收测定法(DEXA)检测的T值进行分组,当腰椎或者股骨T值≥-1.0时记为骨量正常组,-2.5<T<-1.0记为骨量减少组,T≤-2.5记为骨质疏松组。对比各组一般资料,NLR、RDW及MPV的水平,分析各指标间的相关性。结果骨质疏松组的年龄、女性比例均分别明显高于骨量正常组及骨量减少组,BMI水平明显低于骨量正常组及骨量减少组,差异有统计学意义(P<0.05)。骨质疏松组的NLR、RDW及MPV水平均分别明显高于骨量正常组及骨量减少组,且骨量减少组的NLR、RDW及MPV水平均分别明显高于骨量正常组,差异有统计学意义(P<0.05)。根据Spearman法分析相关性发现,患者的年龄、性别与NLR、RDW及MPV水平呈正相关,而BMI与NLR、RDW及MPV水平呈负相关。结论 T2DM并发骨质疏松患者NLR、RDW及MPV水平明显上升,且患者的NLR、RDW及MPV与其年龄、性别及BMI之间具有较为紧密的关联。Objective To study the relationship between levels of NLR, RDW, MPV and osteoporosis in patients with type 2 diabetes mellitus. Methods 314 patients with T2DM treated in this hospital during January 2015 to January 2017 were enrolled in this study. They were grouped according to T value of DEXA detection. Patients with lumbar spine or femoral T value ≥ - 1.0 were listed in normal bone mass group, and patients with -2.5 〈 T 〈 -1.0 were listed in osteopenia group, and paients with T≤ -2.5 were listed in osteoporosis group. The general data and levels of NLR, RDW and MPV were compared between these 2 groups and their correlation among these indicators had been analyzed. Results The age range and the proportion of females in osteoporosis group were significantly higher than those of normal bone mass group and bone mass reduction group, the level of BMI was significantly lower than that of normal bone mass group and bone mass reduction group, and the differ- ence was statistically significant ( P 〈 0.05 ). The levels of NLR, RDW and MPV in pattients of osteoporosis group were significantly higher than those of patients in normal bone mass group and bone mass loss group. The levels of NLR, RDW and MPV in patients of bone mass reduction group were significantly higher than those of normal group, and the difference was statistically significant ( P 〈 0.05 ). According to Spearman analysis, the age, sex and levels of NLR, RDW and MPV were positively correlated, while BMI was negatively correlated with levels of NLR, RDW and MPV. Conclusion The levels of NLR, RDW and MPV were significantly increased in patients with T2DM complicated with osteoporosis, and levels of NLR, RDW and MPV in patients with osteoporosis were closely related to their age, sex and BMI.
