抗HLA抗体对肾移植患者移植肾功能的影响研究

摘　　要：目的研究肾移植术后抗HLA抗体对移植肾存活的影响。方法选择首都医科大学附属北京友谊医院可追踪到的肾移植术后患者78例,检测抗HLA抗体。抗HLA抗体检测方法采用美国One lambda公司提供LABScreen Mixed试剂盒(One Lambda,Inc.,Canoga Park,CA)进行检测,观察抗HLA抗体对移植肾功能的影响。结果 78例肾移植患者中14例为HLA-I类抗体阳性,20例为HLA-II类抗体阳性,22例抗HLA-I+II类抗体阳性。肾功能正常患者分别为6例、8例和2例;肾功能下降或丧失患者分别为8例、12例和20例。肾移植患者术后捡测抗HLA抗体,其结果阳性与阴性对移植肾功能的影响差异有统计学意义(P<0.05)。结论抗HLA抗体对肾移植患者术后肾功能的影响有重要意义。Objective To study the effect of anti-HLA antibodies on survival implication of graft after renal transplantation. Methods A total of 78 patients after renal transplantation could be followed-up in this hospital during October 2014,and levels of anti-HLA antibodies were detected by using LAB screen mixed kits produced by One Lambda Company of USA. Results 14 cases of recipients were positive for HLA class I antibodies,20 recipients were positive for HLA class II antibodies and 22 recipients were positive for HLA class I ＋ II antibodies,and they were detected by using Luminex method. Patients with normal renal function were 6,8 and 2 cases respectively; patients with renal dysfunction or loss of renal function were 8,12 and 20 cases respectively. The difference in effect of anti-HLA antibodies on function of renal grafts in patients with positive or negative result had statistical significance. Conclusion Anti-HLA antibodies can affect the renal function with decline or loss of renal function,hence they are most important factors for successful renal transplantation.
