脑脊液中TNF-α、NSE水平对儿童病毒性脑炎影响的Meta分析

摘　　要：目的系统评价脑脊液中肿瘤坏死因子-α(TNF-α)、神经元特异性烯醇化酶(NSE)与我国儿童病毒性脑炎(VE)的关系。方法在中国知网(CNKI)、万方数据知识服务平台、Cochrane Library、PubMed数据库检索已发表的关于我国VE患儿脑脊液中NSE及TNF-α对VE影响的相关文献,检索年限均从建库截止至2016年11月1日。筛选出VE患儿(VE组)与非VE患儿(对照组)脑脊液中TNF-α和NSE水平的病例-对照研究,对纳入的文献进行质量评价和资料提取,采用RevMan 5.3进行Meta分析。结果共纳入9篇病例对照研究,包括VE组536例,对照组284例。结果显示:VE组患儿脑脊液中TNF-α、NSE水平均显著大于对照组,差异有统计学意义[SMD=1.75,95%CI:1.56~1.93,P<0.01;SMD=1.34,95%CI:1.17~1.51,P<0.01]。结论脑脊液中TNF-α、NSE水平与我国儿童VE的发生呈正相关。Objective To systematically evaluate the influence of TNF-αand NSE levels in cerebrospinal fluid on Chinese children with viral encephalitis（VE）.Methods Literature about the influence of TNF-αand NSE levels in cerebrospinal fluid on Chinese children with VE,which were published before Nov.1,2016,were searched in databases including CNKI,Wanfang Data,Cochrane Library and PubMed.And the case-control study about the TNF-αand NSE levels in cerebrospinal fluid between Children with VE（VE group）and children without VE（control group）were included in this study,and the quality evaluation was implemented and the data were extracted.And then the RevMan 5.3was used to carry out the meta-analysis.Results Nine case-control studies were included in this study,involving 536 cases in the VE group and 284 cases in the control group.The results showed that the level of TNF-αand NSE in cerebrospinal fluid of the VE group were all significantly higher than that of the control group,and the differences was statistically significant[SMD=1.75,95%CI：1.56-1.93,P〈0.01;SMD=1.34,95%CI：1.17-1.51,P〈0.01].Conclusion The levels of TNF-αand NSE in cerebrospinal fluid are positively correlated with the incidence of VE in Chinese children.
