摘 要:目的探讨喹硫平治疗分裂样精神障碍患者的不良反应及其与血药浓度的相关性。方法对70例服用喹硫平治疗的住院分裂样精神障碍患者分别于治疗前和后的第1、2、4、8周末采用副反应量表(TESS)等进行安全性评定,次日晨抽肘静脉血以高效液相色谱法测定其血药浓度。结果喹硫平的不良反应发生率最高为心动过速(34.28%),最低为EPS(2.85%),对体重的影响是双重的,体重增加者为15.71%,体重减轻者为18.57%,各种不良反应的程度多数为轻、中度,其发生时间是在治疗的第4周末,药物达治疗量时,血药浓度与TESS总分值无显著相关(r=0.161~0.232,P>0.05),不同血药浓度不良反应的发生率差异亦无统计学意义。结论喹硫平血药浓度与不良反应无显著相关性,服药剂量在用药的第4周末与TESS总分呈显著正相关关系。提示临床医生在药物加量时,应密切观察不良反应。OBJECTIVE To explore the relationship between side effects of quetiapine and serum concentration in sehizophrenifom disorder. METHODS Seventy schizophrenics treated with quetiapine were included in the stud- y. Treatment e-mergent symptom scale( TESS) was used to assess side effects before and at 1,2,4,8 weeks after treat- ment. Serum quetiapine concentration was determined with high performance liquid chromatography in the next day following the clinical assessment. RESULTS The highest incidence rate of side effects was tachycardia(34. 28% ) and the lowest one was EPS(2. 85% ). Gainingweight ( 15.71% ) and losingweight ( 18.57% ) both emerged. Serum concentration had significant positive correlation with TESS score at the fourth weekend when serum concentration was stable, but the in-cidence rates of side effects had no significant difference in different serum concentra- tion. CONCLUSION Side effect is close related to serum concentration. The determination of serum concentration has important value to evaluate safety of the drug.
