气囊止血带个体化充气压力值设定应用于断指再植术的临床研究

摘　　要：目的:探讨气囊止血带个体化充气压力值设定在断指再植术中的应用效果。方法:选取2014年1月至2016年10月在广东湛江中心人民医院进行断指再植手术患者50例为研究对象,随机分为观察组与对照组,每组25例。对照组气囊止血带充气压力值设定压力为成人上肢30~40 kPa,小儿上肢15~20 kPa;观察组取3次血氧监测仪的动脉波形消失成直线时充气压力的均数为患者个体化气囊止血带压力值。观察2组患者术中止血效果、术后止血带压迫处皮肤情况及患者术后的自觉满意度。结果:观察组患者术中止血效果优于对照组(Hc=17.887,P<0.01);术后止血带压迫处皮肤情况优于对照组(Hc=24.441,P<0.01);术后自觉满意度亦优于对照组(Hc=3.512,P<0.05)。结论:气囊止血带个体化充气压力值设定有利于达到较理想的术中止血效果,改善术后止血带压迫处皮肤情况,提升患者的满意度,值得临床推广。Objective： To investigate the application of individual inflation pressure setting of pneumatic tourniquet in replantation of severed finger. Methods： A total of 50 cases of patients with replantation of severed finger were randomly divided into the observation group and the control group, 25 cases in each group. Inflation pressure of pneumatic tourniquet in the control group was 30-40 kPa for adult upper limb and 15-20 kPa for pediatric upper limb. The inflation pressure when the arterial waveform of the oxygen monitor became a straight line was measured three times, and the mean value was calculated in the observation group. The intraoperative hemostatic effect, the postoperative skin condition of tourniquet compression, and the postoperative satisfaction were compared between the two groups. Result： The intraoperative hemostatic effect, the postoperative skin condition, and the postoperative satisfaction in the observation group were all better than those in the control group （Hc=17.887, P〈0.01, Hc= 24.441, P〈0.01, Hc=3.512, P〈0.05, respectively） . Conclusion： The appropriate individual inflation pressure of pneumatic tourniquet can achieve the desired hemostatic effect, improve the skin condition of tourniquet compression, and enhance the satisfaction of patients, which is worthy of clinical promotion.
