CD4＋CD25＋Foxp3＋调节性T细胞及相关细胞因子在类固醇抵抗型变应性鼻炎患者外周血中的变化及意义

摘　　要：目的探讨类固醇激素抵抗型变应性鼻炎（AR）患者外周血CD4^＋CD25^＋Foxp3^＋调节性T细胞（Treg）及相关细胞因子白介素-10（IL-10）、转化生长因子-β1（TGF—β1）水平的变化，分析其在类固醇抵抗型AR发病中的作用。方法采用流式细胞术检测30例健康体检者（健康对照组）、30例激素敏感AR患者（激素敏感组）及30例类固醇抵抗型AR患者（激素抵抗组）外周血CD4^＋CD25^＋Foxp3^＋Treg占CD4＋T淋巴细胞的百分比，酶联免疫吸附实验检测该三组研究对象血清IL-10、TGF-β1水平。结果与健康对照组比较，激素敏感组及激素抵抗组外周血CD4^＋CD25^＋Foxp3^＋Treg占CD4＋T的百分比、血清IL-10与TGF-β1水平明显降低（P〈0．01）；与激素敏感组比较，激素抵抗组外周血CD4^＋CD25^＋Foxp3^＋Treg／CD4＋T、血清IL-10与TGβ1水平亦明显降低（P〈0．01）；外周血CD4^＋CD25^＋Foxp3^＋Treg／CD4＋T与血清IL-10、TGF-β1水平呈正相关（P〈0．01）。结论类固醇抵抗型AR患者外周血CD4^＋CD25^＋Foxp3^＋Treg数目减少，相关细胞因子IL-10、TGF-β1含量亦降低，提示该细胞亚群及相关细胞因子在类固醇抵抗型AR的形成中发挥重要作用。Objective To investigate the changes of CD4^＋CD25^＋Foxp3^＋ regulatory T cell （Treg） and relative cell factors in peripheral blood of steroid-resistant allergic rhinitis （AR） patients, and to study their functions in occurrence of steroid-resistant AR. Methods The CD4^＋CD25^＋Foxp3^＋ Treg cell in 30 cases general physical examination （The control group）, 30 cases steroid-sensitive AR patients （ steroid- sensitive group） and 30 cases steroid-resistant AR patients （steroid-resistant group） were detected by flow cytometry, The levels of serum interleukin 10 （IL-10） and transforming growth factor β1 （TGF-β1） in the three groups were detected by enzyme-linked immunoadsorbent assay （ELISA）. Results Compare to the control group, proportion of CD4^＋CD25^＋Foxp3^＋ Treg cell and the levels of serum IL-10, TGF-β1 in ster- oid-sensitive group and steroid-resistant group were significantly reduced （ P 〈 0. 01 ）. The proportion of CD4^＋CD25^＋Foxp3^＋ Treg cell and the levels of serum IL-10, TGF-β1 in steroid-resistant group were decreased significantly than steroid-sensitive group （ P 〈 0. 01 ）. The proportion of CD4^＋CD25^＋Foxp3^＋ Treg cell were positively correlated with the serum IL-10, and TGF-β1 （P 〈 0. 01 ）. Conclusions The reduced proportion of CD4^＋CD25^＋Foxp3^＋Treg cell and lower serum IL-10, TGF-I31 play an important role in the occurrence of steroid-resistant AR patients.
