阵发性心房颤动与房性早搏的相关性分析

摘　　要：目的通过动态心电图检查评价阵发性心房颤动(PAF)与房性早搏之间的相关性。方法 PAF的患者80例,统计24 h心房颤动发作的次数,并观察心房颤动发作前有无触发性房性早搏,测定早搏指数(PI)及偶联间期(CI),并观察心房颤动发作前的长-短周期现象。结果 PAF共发作384次,其中242次心房颤动发作前可见触发性房性早搏(63.0%),其早搏指数及联律间期明显小于/短于非触发性房性早搏。55.6%的PAF发生前存在长-短周期现象。结论 PAF大多有房性早搏驱动,且触发性房性早搏的早搏指数更小,联律间期更短,相当一部分PAF发作时存在长短周期现象。Objective To evaluate the association between paroxysmal atrial fibrillation（PAF） and atrial premature beats by dynamic electrocardiographic examination. Methods Eighty patients with PAF were enrolled in this study. The number of episodes of atrial fibrillation was measured at 24 h and observed whether their had triggering atrial premature beats before the onset of atrial fibrillation. At the same time,the premature beat index（PI） and coupling interval（CI） of these patients were also measured and the long and short cycle phenomenon before the onset of atrial fibrillation was observed. Results PAF had a total of 384 episodes,of which 242 cases has triggering atrial premature beats before the onset of atrial fibrillation（63.0%）,the premature beat index（PI） and coupling interval（CI） were significantly less than non-triggering atrial premature beats. Among them,55.6% of PAF which was before the occurrence,could be observed long and short cycle phenomenon. Conclusion There are many cases of paroxysmal atrial fibrillation which has the tendency of atrial premature beats. In triggering atrial premature beats,the PI is smaller and the CI is shorter. There is a considerable part of paroxysmal atrial fibrillation seizures of the existence of long and short cycle phenomenon.
