老年2型糖尿病患者血清雌二醇水平和颈动脉硬化的相关性

摘　　要：目的:探讨老年2型糖尿病患者血清雌二醇水平和颈动脉硬化的相关性。方法:选择2013-08-2016-01在我院进行诊治的老年2型糖尿病患者182例为观察组,同期选择在我院进行体检的健康老年人(≥60岁)182例为对照组,2组都进行性激素(雌二醇、睾酮)及常规代谢指标(TC、TG、LDL、Glu、HDL)的测定,同时采用超声观察颈动脉硬化状况与相关性分析。结果:观察组的血清TC、TG、LDL、Glu含量显著高于对照组,HDL含量显著低于对照组,2组比较差异有统计学意义(P<0.05)。观察组血清雌二醇与睾酮含量分别为(119.49±16.29)nmol/L和(18.34±2.47)nmol/L,对照组分别为(160.35±17.24)nmol/L和(11.42±3.11)nmol/L,2组比较差异有统计学意义(P<0.05)。在观察组中,血清雌二醇含量与HDL呈现明显正相关性(P<0.05),而与TC、TG、LDL、Glu、内中膜厚度(IMT)呈明显负相关(P<0.05);多重线性回归分析显示雌二醇、LDL、Glu为导致IMT增厚的主要独立危险因素(P<0.05)。结论:老年2型糖尿病患者血清雌二醇水平呈现低表达和颈动脉硬化状况,可加重糖脂代谢紊乱,促进糖尿病的发生、发展。Objective：To investigate the relationship between serum estradiol and carotid atherosclerosis in elderly patients with type 2diabetes mellitus.Method：From August 2013 to January 2016,182 elderly patients with type 2diabetes mellitus were selected in our hospital for diagnosis and treatment as the observation group,and 180 health elderly cases were selected at the same period as the control group,sex hormone and metabolic indicators of two groups were measured,and the carotid atherosclerosis was observed and the correlation analysis was carried on.Result：The serum levels of TC,TG,Glu and LDL in the observation group were significantly higher than those in the control group,the HDL level was significantly lower than that of the control group（P〈0.05）.The serum levels of T and E2 in the observation group were（18.34±2.47）nmol/L and（119.49±16.29）nmol/L respectively,the control group were（11.42 ±3.11）nmol/L and（160.35±17.24）nmol/L respectively,and the differences between the two groups were statistically significant（P〈0.05）.In the observation group,the serum levels of E2 and HDL were showed significant positive correlation（P〈0.05）,and TC,TG,LDL,Glu and IMT were showed negatively correlated（P〈0.05）.Multiple linear regression analysis showed that E2,LDL and Glu were as the leading independent risk factors of IMT thickening（P〈0.05）.Conclusion：Elderly patients with type2 diabetes mellitus have the low expression level of serum E2 and carotid atherosclerosis,which could aggravate the disorder of glucose and lipid metabolism,and promote the development of diabetes.
