桃核承气汤联合肠梗阻导管治疗粘连性肠梗阻临床研究

摘　　要：目的:观察桃核承气汤联合肠梗阻导管治疗粘连性肠梗阻的临床疗效。方法:将62例粘连性肠梗阻患者按随机数字表法分为A组、B组,每组各21例,C组20例。三组患者均采用西医保守治疗,A组加用桃核承气汤[桃仁15 g,桂枝9 g,生大黄(后下)15 g,甘草6 g,芒硝(冲)6 g]联合肠梗阻导管治疗;B组采用肠梗阻导管减压法治疗;C组采用鼻胃管减压法治疗。观察两组患者临床疗效,腹痛腹胀消失时间,中转手术比率,以及白细胞计数、C反应蛋白(C-reactionprotein,CRP)、肿瘤坏死因子-α(Tumor necrosis factor alpha,TNF-α)水平。结果:治疗后A组患者腹痛、腹胀等临床症状缓解迅速,中转手术比率下降,在治疗第5天起白细胞计数、CRP、TNF-α水平显著下降,与B组、C组比较,差异均有统计学意义(P<0.05)。结论:桃核承气汤联合肠梗阻导管治疗粘连性肠梗阻临床疗效显著。Objective：To observe the clinical curative effects of adhesive intestinal obstruction treated by Taohe Chengqi decoctioncombined with intestinal obstruction catheter. Methods：62 patients were randomly divided into three groups,in which the patientsin A group （n = 21 ） were treated by Taohe Chengqi decoction combined with intestinal obstruction catheter, the patients in Bgroup （ n = 21 ） were treated with intestinal obstruction catheter, and the patients in C group （ n = 20） were treated with nasogastrictube. Composition of Taohe Chengqi decoction ： Taoren （Peach） 15 g, Guizhi （ Cassia Twig） 9 g, Shengdaihuang （ Raw Rhubarb） 15g （later）, Gancao （Licorice） 6 g, Mangxiao （Mirabilite） 6 g.（Dissolving）. The clinical curative effects were observed, and the timeof the abdominal pain, abdominal distension disappears, the ratio of changed into operation, white blood cell count and the levofCRP,TNF-ct were observed. Results：Clinical symptom such as abdominal pain and distension of the patients in A group were re-lieved quickly, the rate of changed into operation declined. On the 5th day of treatment, the leukocyte count, the level of RP andTNF-ot of them was dropped significantly. Compared with the patients in group B and group C ,it had statistical significance（ P 〈0. 05 ）.
