静脉联合局部应用氨甲环酸对行人工全髋关节置换术患者术后指标的影响

摘　　要：目的:探讨静脉联合局部应用氨甲环酸对行人工全髋关节置换术患者术后失血量及血红蛋白水平的影响。方法:选取2014年3月至2016年4月行人工全髋关节置换术的患者68例,随机分为观察组和对照组,各34例。对照组静脉应用氨甲环酸,观察组静脉联合局部应用氨甲环酸。统计对比两组患者术后失血情况、术前及术后第2 d血红蛋白及红细胞压积水平变化情况及并发症发生率。结果:观察组术后失血量、隐性失血量及总失血量、术后引流量少于对照组(P<0.05);术前两组血红蛋白及红细胞压积比较,差异无统计学意义(P>0.05),术后第2 d两组各指标均较术前降低,但观察组血红蛋白及红细胞压积高于对照组(P<0.05)。结论:静脉联合局部应用氨甲环酸可有效减少行人工全髋关节置换术患者术后失血量,维持血红蛋白处于较高水平,且安全性较高。Objective： To investigate the influence of venous and local application of tranexamic acid on postoperative blood loss and hemoglobin levels of patients with total hip replacement. Methods： 68 cases of total hip replacement from March 2014 to April 2016 were selected and divided into the observation group and the control group,34 cases in each group. The control group was treated with tranexamic acid while the observation group was treated with venous and local application of tranexamic acid,with the postoperative bleeding,hemoglobin and hematocrit level changes before and 2 d after surgery and the occurrence rate of complications compared. Results： Postoperative blood loss,hidden blood loss,total blood loss,postoperative drainage of the observation group were less than those of the control groups（ P〈0. 05）; There was no difference in hemoglobin and hematocrit before the surgery,the difference being not significant（ P〉0. 05）; 2d after the surgery,all the indexes of the two groups declined,but hemoglobin and hematocrit of the observation group were higher than those of the control group（ P〈0. 05）; The occurrence rates of complications in the observation group and control group were 5. 88 %（ 2/34） and 8. 82 %（ 3/34） respectively,the difference being not significant（ P〉0.05）. Conclusion： Venous and local application of tranexamic acid can lessen postoperative blood loss of patients with total hip replacement,maintain hemoglobin at high level and has higher safety.
