摘 要:目的:观察外科坐浴方结合窄谱红光照射对低位单纯性肛瘘术后创面的临床疗效及其安全性。方法:采用分层区组随机的研究方法,将120例低位单纯性肛瘘术后患者随机分为对照组(外科坐浴方)和治疗组(外科坐浴方结合窄谱红光照射)。通过观察术后患者不同时期创面疼痛情况、创面渗液情况、创面愈合率,治疗前后有无出现不良反应等总结出综合疗法的临床疗效及其安全性。此外,从对照组和治疗组随机抽出25例于第7天、第14天取创面肉芽组织作病理切片分析,从分子水平观察镜下创面成纤维细胞数目情况。结果:与对照组比较,治疗组在术后第7天、第14天的创面疼痛评分、水肿评分、渗液评分以及愈合率比较均有明显统计学意义(P<0.05),治疗组术后7 d成纤维细胞数目多于对照组(P<0.05),术后14 d却明显少于对照组(P<0.05)。结论:低位单纯性肛瘘术后创面应用外科坐浴方结合窄谱红光照射能明显减轻术后创面疼痛、促进创面愈合,缩短术后创面愈合时间,值得临床推广。Objective : To observe the clinical effect and safety of Surgery Sitz Bath Decoction combined with narrow-spectrum red glow in the treatment of anal fistula wounds. Methods : 120 cases of simple low anal fistula patients were randomly divided into control group and treatment group by research method of stratified area group. The clinical efficacy and safety of the combined therapy were summarized by observing the pain of the patients in different stages, the infiltration of the wound, the rate of wound healing, and the adverse reactions before and after treatment. In addition, 25 patients were randomly selected from the control group and the treatment group on the 7 d, 14 d of the wound granulation tissue for pathological section analysis, the number of fibroblasts under the microscope from the molecular level were recorded. Results : Compared with the control group, the treatment group at 7 d after operation, the 14 d wound pain score, edema score, effusion score and healing rate were statistically significant (P〈0.05), the treatment group 7 d after operation, the number of fibroblast cells than in the control group (P〈0.05), 14 d after operation was significantly less than the control group ( P〈0.05 ). Conclusion : Surgery Sitz Bath Decoction combined with narrow-band red light irradiation in the treatment of simple low anal fistula can significantly reduce postoperative wound pain, promote wound healing, shorten the time of wound healing after surgery, which is worthy of clinical application.
