摘　　要：目的系统评价通天口服液联合钙拮抗剂治疗偏头痛的有效性和安全性。方法计算机检索Pub Med、CBM、Wan Fang Data、VIP和CNKI等数据库,搜索通天口服液、钙拮抗剂、通天口服液联合钙拮抗剂治疗偏头痛的相关随机对照试验(RCT),检索时限均从建库至2015年11月。由2位评价员独立筛选文献、提取资料并评价纳入研究的偏倚风险后,应用Rev Man 5.2软件进行meta分析。结果共纳入14个RCT,包括1 215例患者。meta分析结果显示,与钙拮抗剂组相比,通天口服液治疗偏头痛的显效率更高(OR=1.53,95%CI 1.14~2.04,P=0.004)。与单用通天口服液或者单用钙拮抗剂相比,通天口服液联合钙拮抗剂治疗偏头痛的显效率均较高(OR=3.96,95%CI 2.72~5.78,P<0.000 01;OR=3.67,95%CI 2.03~6.64,P<0.000 1)。与单一用药相比,两药联用未增加不良反应发生率。结论循证医学分析表明通天口服液联合钙拮抗剂治疗偏头痛临床疗效优于单一使用钙拮抗剂或通天口服液。Objective To systematically evaluate the efficacy and safety of Tongtian Oral Liquid combined with calcium antagonists in treating hemicrania. Methods The databases of Pub Med,CBM,Wan Fang Data,VIP and CNKI were retrieved by computer for searching the related randomized controlled trials（RCT） on Tongtian Oral Liquid combined with calcium antagonists in treating hemicrania,the retriveal time limitation was from their establishment to Nov. 2015. The two evaluators indepen-dently screened the literatures,extracted the data and evaluated the bias risk of included RCT. Then the meta analysis was per-formed by using the Rev Man 5.2 software. Results A total of 14 RCT involving 1 215 patients were included. The meta analysis results demonstrated that compared with the calcium antagonists group,Tongtian Oral Liquid in treating hemicrania had higher significantly effective rate（OR=1.53,95%CI 1.14-2.04,P=0.004）. Compared with the single Tongtian Oral Liquid or single calcium antogonists,Tongtian Oral Liquid combined with calcium antagonists in treating hemicrania had higher significantly effective rate（OR=3.96,95% CI 2.72-5.78,P〈0.000 01; OR=3.67,95%CI 2.03-6.64,P〈0.000 1）. Compared with single medication,the2-drug combined use did not increase the adverse reactions. Conclusion The evidence-based medicine analysis shows that the clinical effect of Tongtian Oral Liquid combined with calcium antagonists in treating hemicrania is superior to single use ofcalci-um antagonists or Tongtian Oral Liquid.
