腓骨骨皮瓣移植联合负压封闭引流治疗胫骨缺失研究

摘　　要：目的探讨分析胫骨慢性骨髓炎伴骨缺失患者采用带血管蒂腓骨骨皮瓣移植联合负压封闭引流的临床效果。方法从本院2014年1月—2016年1月收治的胫骨慢性骨髓炎伴骨缺失患者中随机选取20例作为研究对象,所有患者均采用带血管蒂腓骨骨皮瓣移植治疗,而后采用负压封闭引流技术将创面覆盖,观察治疗后的临床效果。结果 20例患者经过观察、随访显示,所有患者窦道在4周内愈合,移植的皮瓣均成功存活,经过X线检查显示20例患者移植腓骨也全部愈合,愈合时间(5.1±0.6)月。结论临床治疗胫骨慢性骨髓炎伴骨缺失患者采用带血管蒂腓骨骨皮瓣移植联合负压封闭引流能有效提高治疗效果,修复骨缺损,恢复患肢功能,同时还能够有效控制感染。Objective To investigate the clinical effect of vascularized fibular bone flap combined with vacuum sealing drainage in the treatment of chronic osteomyelitis of tibia with bone defect. Methods From January 2014 to January 2016, 20 patients with chronic osteomyelitis accompanied by bone loss were selected as the subjects in our hospital, they were treated with vascularized fibular bone flap, negative pressure closed drainage technique was used to cover the wound, the clinical effect was observed after treatment. Results 20 cases of patients after observation and followup showed that all patients were healed within 4 weeks, the transplanted flaps survived, after X-ray examination, it showed that 20 patients were all transplanted fibular healing, healing time was（5.1±0.6） months. Conclusion The clinical treatment of chronic osteomyelitis with bone loss in patients with tibial transplantation combined with vacuum sealing drainage can effectively improve the therapeutic effect of vascularized fibular bone flap to repair bone defects, restore limb function, also can effectively control the infection.
