艾滋病患者NK细胞数量与疾病进展及HAART免疫应答相关性研究

摘　　要：目的探讨HIV感染患者NK细胞与疾病进展及HAART免疫应答相关性。方法收集2011年4月-2014年12月HIV感染患者53例,对照组26例;根据HIV感染患者CD4+T淋巴细胞数量分为HIV组和AIDS组;根据HAART时间分为HAART前组、治疗12个月组;根据HAART免疫应答分为有效组和受损组。结果 HIV组和AIDS组NK细胞数量明显低于对照组,AIDS组NK细胞数量明显低于HIV组,差异有统计学意义(P<0.05);当NK细胞数量取252.5个/μl时,HIV/AIDS疾病分期的敏感度、特异度分别为71.0%、63.6%。艾滋病患者随着HAART的进行,CD4+T淋巴细胞和NK细胞数量显著增高,HAART前组、治疗12个月组和对照组间差异有统计学意义(F=5.72,P<0.05);当NK细胞数量取237.0个/μl时,预判免疫应答有效的敏感度、特异度分别为74.3%、55.6%。结论 HIV感染后NK细胞与疾病进展相关,HAART可部分恢复NK细胞,治疗前NK细胞水平与HAART免疫应答相关。Objective To investigate the correlation among in the count of natural kill（ NK） cells of HIV infected patients and disease progression as well as the immune response of HARRT. Methods 53 cases of patients with HIV infection from April2011 to December 2014 were collected and 26 healthy volunteers were collected as control group. According to the count of CD4＋T lymphocytes,patients were divided into HIV and AIDS groups. According to the treat time of HAART,patients were divided into before-the-HAART-treatment group（ BT） and 12-months-after-the-treatment group（ AT）. According to immune-response of HAART,patients were divided into effective and damaged groups. Results The number of NK cells in HIV group and AIDS group was significantly lower than that in control group,the number of NK cells in AIDS group was significantly lower than that in group HIV,and the difference was statistically significant（ P〈0. 05）. When the NK cell count before the treatment was 252. 5 one/μl,the sensitivity and specificity on the staging of HIV/AIDS were 71. 0% and 63. 6%,respectively. With the development of HAART,the number of CD4＋T lymphocytes and NK cells increased significantly. There was significant difference between before-the-HAART-treatment group and 12-months-after-the-treatment group（ F =5. 72,P〈0. 05）. When the NK cell count before the treatment was 237. 0 one/μl,the sensitivity and specificity on prediction of the immune response effective of HARRT,were 74. 3% and 55. 6%,respectively. Conclusion The NK cell counts after HIV infection were related to disease progression. The counts of NK cell were restored partly by the treatment of HAART. The levels of NK cell count before treatment were related to the immune response of HARRT.
