摘 要:目的分析重症监护病房铜绿假单胞菌感染的药敏变化,并比较中心重症监护病房(ICU)、呼吸重症监护病房(RICU)、脑外科重症监护病房(NSICU)与急诊重症监护病房(EICU)4个病区之间铜绿假单胞菌的耐药性,为临床科学合理的使用抗菌药物提供依据。方法收集2013年1月-2016年12月温州医科大学附属第一医院4个重症监护病房送检标本中所分离的铜绿假单胞菌共506株,其中ICU分离211株,RICU分离108株,NSICU分离127株,EICU分离60株。分析4个病区分离的铜绿假单胞菌的耐药性是否存在差异。结果在11种临床常用抗菌药物中,铜绿假单胞菌对亚胺培南的耐药率最高(50.99%),对阿米卡星的耐药率最低(12.45%)。NSICU耐药情况最严重,RICU、ICU次之,EICU耐药情况较轻。ICU、RICU、NSICU和EICU多重耐药菌株的分离率差异无统计学意义(P>0.05)。结论NSICU和RICU分离菌分离的铜绿假单胞菌相对于ICU和EICU的分离株耐药情况更严重。Objective To analyze the drug sensitivity of Pseudomonas aeruginosa( PAE) isolated from central Intensive Care Unit( ICU),and compare the drug resistance of PAE in ICU,the Respire Intensive Care Unit( RICU),Nerve Surgery Intensive Care Unit( NSICU) and Emergency Intensive Care Unit( EICU),so as to provide scientific basis for rational use of antibiotics in clinical practice. Methods 506 PAE isolates were isolated from ICU( n = 211),RICU( n = 108),NSICU( n = 127) and EICU( n = 60) at the first affiliated hospital of Wenzhou Medical University from January 2013 to December 2016. The difference of drug resistance of PAE isolated from 4 wards was analyzed. Results The highest resistance rate of PAE to 11 antimicrobial agents was imipenem( 50. 99%),the lowest resistance rate was amikacin( 12. 45%). NSICU drug resistance was the most severe,followed by RICU and ICU,and the drug resistance of EICU was the lightest. There were no statistical significance on the differences in the separation rate of multiple drug resistant strains of ICU,RICU,NSICU and EICU( P〉0. 05). Conclusion The drug resistance of PAE isolated from NSICU and RICU was more serious than those from ICU and EICU.
