规范化围术期综合评估图(量)表在日间手术模式实施中的应用

摘　　要：我国目前医疗优势资源大多集中在大中型医院(区域医疗中心),而这些优势资源却因患者住院时间较长而未被高效使用,加上患者愿意选择到大医院看病,致使这些大医院出现"看病难、住院难、手术迟"的现象。日间手术模式因住院时间短越来越受到国人的青睐,是一种可能解决上述矛盾的新的医疗模式,但因患者住院时间短,对其围术期正确评估显得尤为重要,为最大限度消除医疗隐患,笔者深究中国《日间手术麻醉专家共识》之精髓,逐渐制作出具有我院特色较规范的、可操作性强的围术期综合评估及术后随访图(量)表,这些可能对初始开展日间手术的医院格外有意义,甚至可能更有利于日间手术模式在基层得以推广。Most of the medical resource is concentrated in large and medium-sized hospitals （regional medical center） in China and the resource is not being used efficiently to maximize these resources due to the length of hospital stay was too long.Most of the patients preferring the ＂better hospital＂ （the larger hospital） make hospital overloaded and patients are hard to get admitted.All of this resulted in difficulty to receive high-quality medical care, hospitalization, and surgery.It is the common mission of all Chinese medical and nursing staff to reduce medical costs and provide patients with a safe, fast, comfortable and economic service through integrating the existing medical resources.By simplifying the clerical process of medical treatment, shortening the waiting time and the length of stay in hospital, and make hospital beds more available.Patients that may possibly be a solution of the difficult to receive medical care, hospitalization and surgery, favor ambulatory surgery as a new medical model.By completing the admission to hospital, surgery and hospital discharge in one day, which provides shorter hospital stay.It requires the high level of the surgeon, especially requires the high-quality anesthesiologist to started this new-model medical serves in our country.We have reached a consensus on the ambulatory surgery anesthesia, summarized the experience and explore the proper ambulatory surgery mode and management processes in the regional medical center.Which aims to eliminate medical risks and ensure the safety of perioperative patients when receiving ambulatory surgery.Meanwhile, we have improved the perioperative evaluation and postoperative follow-up system to the standardized operable charts and scales.Those can all be advantageous to the hospitals starting ambulatory surgery and promoting the ambulatory surgery in the primary hospitals.
