摘 要:目的:观察接骨七厘片联合负压封闭引流技术(VSD)治疗跗跖关节骨折脱位合并严重软组织损伤的临床疗效。方法:将跗跖关节骨折脱位合并严重软组织损伤的70例患者随机分为对照组与治疗组各35例,两组均采用解剖复位内固定术治疗。对照组术后进行常规创面换药、抗感染,治疗组术后口服接骨七厘片并创面负压封闭引流。结果:两组患者术后7d疼痛、肿胀及瘀斑等症状评分及ESR、CRP等指标水平较术前降低,差异具有统计学意义(P<0.05),治疗组术后7d血浆黏度较术前降低(P<0.05),治疗组术后7d疼痛、肿胀及瘀斑等症状评分及血浆黏度、ESR、CRP等指标水平降低幅度大于对照组,差异具有统计学意义(P<0.05);治疗组软组织损伤术后7d总有效率及跗跖关节骨折脱位术后28d总有效率高于对照组,差异具有统计学意义(P<0.05)。结论:接骨七厘片联合负压封闭引流技术能够显著改善跗跖关节骨折脱位合并严重软组织损伤的临床症状及临床理化指标水平,有助于促进软组织损伤的恢复跗跖关节骨折脱位的愈合。Objective: To observe the clinical effect of jie gu qi li pill combined with negative pressure occlusion technique( VSD) in the treatment of tarsal joint fractures with severe soft tissue injury. Methods:70 patients with severe tarsus joint fracture and dislocation combined with severe soft tissue injury were randomly divided into control group and treatment group( 35 cases for each group). Both groups were treated by anatomical reduction and internal fixation. The control group were treated with conventional wound dressing and anti-infection,while The patients in the treatment group were treated with jie gu qi li pill and negative pressure occlusion technique( VSD). Results: The two groups' scores of pain,swelling and ecchymosis were significantly lower than those before operation( P〈0.05).and the plasma viscosity of the two groups was lower than that before operation( P〈0.05). The effective rate of pain,swelling and ecchymosis,such as ESR and CRP,in the treatment group was significantly higher than that in the control group( P〈0.05). The total effective rate was lower than that of the control group at 28 days after fracture and dislocation( P〈0.05). Conclusion: The combination of jie gu qi li pill combined with negative pressure occlusion technique can significantly improve the clinical symptoms and clinical physical and chemical indexes of tarsal metatarsal joint fracture and dislocation with severe soft tissue injury and help to promote the recovery of soft tissue injury.
