摘 要:目的利用胸部CT扫描轴位图像的四个固定层面胸膜间距离之和计算气胸的肺压缩程度,以探索基于CT轴位图像更简便、精确的气胸评价方法。方法回顾性分析2013年1月至2015年6月间本院73例气胸患者的胸部CT平扫资料,将层厚为2 mm的胸部CT数据导入东芝Aquilion ONE后处理软件Vitrea(Version6.5.8),测得的容积数据经计算得到的肺压缩值作为参考标准;将胸廓入口内缘至压缩肺尖边缘距离记为A,在胸部CT的固定三个解剖层面(气管分叉层面、嵴角层面、膈顶层面)分别标记压缩肺锁骨中线及腋中线的气体厚度(矢状面三线:沿锁骨中线矢状面测量三个固定解剖层面脏壁层胸膜间距离的三线分别记为B、C、D;冠状面三线:沿腋中线冠状面测量三个固定解剖层面脏壁层胸膜间距离的三线分别记为B^1、C^1、D^1),计算距离之和(A+B+C+D)及(A+B^1+C^1+D^1),将所得结果分别与CT容积测量法所测值进行直线回归分析,得出直线回归方程。结果根据肺压缩值与两组CT四线之和的结果回归分析,计算肺萎陷程度的公式为:矢状面三线Y=0.001×(A+B+C+D)+0.234(r=0.855,P<0.001);冠状面三线Y^1=0.002×(A+B^1+C^1+D^1)+0.247(r^1=0.867,P1<0.001)。结论基于常规胸部CT扫描轴位图像测量的CT四线法是计算气胸肺萎陷程度方便而准确的方法,值得临床推广应用。Objective To calculate lung compression degree of patients with pneumothorax by using the distances of 4 fixation level pleura of Travers image of chest CT scan of patients with pneumothorax to explore easier and more accurate pneumothorax evaluation method based on CT axial images. Methods A retrospective analysis of chest CT plain scanning was done on 73 patients with pneumothorax in our hospital from 1st of January 2013 to 30 th of June 2015. Then calculated the lung compression value by volumetric data gained by inputting chest CT data with a layer thickness of 2mm to Toshiba Aquilion ONE post-processing software Vitrea( Version6. 5. 8) as reference standard. Record edge distance from thoracic inlet to compressed apex pulmonis edge as A,respectively mark gas thickness of midclavicular line of compressed lung and midaxillary line( three lines in vertical plane: the three lines that measure distance of 3 fixed dissection level lamina visceral pleura along vertical plane of midclavicular line are respectively regarded as B/C/D; three lines in coronal plane: the three lines that measure distance of 3 fixed dissection level lamina visceral pleura along coronal plane of midaxillary line are respectively regarded as B^1/C^1/D^1) on the three fixed levels of Chest CT( level of tracheal bifurcation,ridge angle level,diaphragm dome level). Calculate the sum of the distance( A + B + C + D) and( A + B^1+ C^1+ D^1),and do linear regression analysis for the obtained results and the values measured by CT measurement method to get linear regression equation. Results According to result regression analysis of sum of lung compression value and two groups of CT four-wire,formula to calculate the degree of lung collapse is: three lines sagittal plane: Y = 0. 001 ×( A + B + C + D) + 0. 234,r = 0. 855,P 0. 001; three lines of coronal line: Y^1 = 0. 002 ×( A + B^1+ C^1+ D^1) + 0. 247,r^1= 0. 867,P^1〈 0. 001.
