摘　　要：目的探讨贮存式自体输血(Preoperative autologous blood donation,PABD)在择期手术中的应用。方法回顾性分析2016年1月-2017年5月我院择期手术进行PABD的1 026例患者的临床资料并与同期未进行PABD的择期手术患者进行比较。结果 (1)我院进行PABD的手术科室主要是神经外科、泌尿外科、骨科、妇科及肝胆外科,分别占同期择期手术患者的5.97%、4.90%、3.78%、3.76%及3.55%;(2)神经外科、泌尿外科、骨科、妇科及肝胆外科择期手术异体RBC输血率,PABD组分别为5.66%、2.51%、4.89%、0及4.88%,非PABD组分别为25.46%、28.58%、24.77%、10.62%及17.54%;(3)术前血红蛋白<120 g/L组异体RBC输血率显著高于(120-129.9)g/L组、(130-139.9)g/L组及≥140 g/L组(7.65%vs 2.44%、3.66%及2.70%,P<0.05)。结论 (1)综合医院开展PABD主要以神经外科、泌尿外科、骨科、妇科及肝胆外科为主;(2)Hb水平<120 g/L的择期手术患者异体RBC输血率高;(3)需要改良PABD才能使患者从PABD中获益。Objective To investigate the application of preoperative autologous blood donation（PABD） in selective op- eration. Methods Retrospective investigations and analysis were carried out in clinical datas of 1 026 patients of PABD in selective operation from January 2016 to may 2017, comparing with those who had not performed PABD in the same time. Results ①The surgeries of PABD were mainly neurosurgery, urology, orthopedics, gynecology and hepatobiliary surgery, respectively, which accounted for 5.97%, 4.90%, 3.78%, 3.76% and 3.55% of the patients in the same period ; ②The rate of transfusion of allogeneic red blood cells of selective operation in PABD group of neurosurgery, urology, orthopedics, gynecology and hepatobiliary surgery were 5.66%, 2. 51%, 4. 89%, 0 and 4. 88%, respetively, while not performed PABD group were 25.46%, 28. 58%, 24. 77%, 10. 62% and 17. 54%, respetively; ③The rate of transfusion of the group of hemo- globin （Hb） 〈120 g/L is significantly higher than those Hb is （ 120-129. 9）g/L, （ 130-139. 9）g/L and ≥ 140 g/L（7.65% vs 2. 44%, 3.66% and 2. 70%, P〉0. 05）. Conclusion ①PABD is mainly carried out in neurosurgery, urology, orthope- dics, gynecology and hepatobiliary surgery; ②The group of Hb〈 120 g/L has a high rate transfusion of allogeneic erythrocyte in selective operations; ③Only the PABD is improved can patients make benefit from it.
