降钙素原、C反应蛋白和心肌酶CK-MB检测在轮状病毒性肠炎患儿中的诊断价值

摘　　要：目的探究降钙素原(PCT)、C反应蛋白(CRP)和心肌酶CK-MB检测在轮状病毒性肠炎患儿中的诊断价值。方法选取住院治疗的轮状病毒性肠炎患儿60例作为观察组,并选取56例细菌性肠炎患儿作为对照组,对其PCT、CRP及CK-MB水平进行检测,比较2组的各指标差异情况。观察组患儿依据脱水情况分为重度脱水组、轻中度脱水组及无脱水组,比较不同脱水情况的PCT、CRP及CK-MB水平。结果观察组患者的PCT、CRP水平,PCT、CRP阳性率普遍低于对照组,差异有统计学意义(P<0.05);CK-MB的水平显著高于对照组,CK-MB的阳性率显著高于对照组(P<0.05)。观察组随着脱水程度加重,CK-MB阳性率逐渐增高,重度脱水组患者CK-MB的阳性率相比于其他组差异有统计学意义(P<0.05);同时比较观察组中不同脱水情况下CK-MB水平,轻中度脱水组CK-MB水平高于无脱水组(P<0.05),重度脱水组CK-MB水平高于无脱水组和轻中度脱水组(P<0.05)。重度脱水组血清CRP水平高于轻中度脱水组及无脱水组(P<0.05)。结论通过PCT与CRP及CK-MB的检测对鉴别婴幼儿轮状病毒性肠炎与细菌性肠炎具有重要意义,结合CRP的水平能够判断轮状病毒性肠炎的严重程度。Objective To study the diagnosis value of PCT, CRP and CK - MB in children with rotavirus enteritis. Methods 60 children with rotavirus enteritis and 56 children with bacterial enteritis were selected as the observated group（ the rotavirus enteritis groups） and the control group （bacterial group）. PCT, CRP and CK - MB levels were detected and compared. Futhermore, the observated group were divided into no dehydration, mild dehydration and severe dehydration group, the level of PCT, CRP and CK - MB were compared. Results Both PCT and CRP in the bacterial enteritis had positive value;CK - MB and CRP of rotavirus enteritis had positive value. The positive rate of CRP and PCT of bacterial group were significantly higher than that in the rotavirus enteritis groups （P 〈0.05） ;the positive rate of CK- MB in rotavirus group was significantly higher than that in bacterial group（ P 〈 0.05 ）, CRP and CK - MB in the dehydration of rotavirus enteritis were increased with the degree of dehydration. Conclusion PCT and CRP can be used to distinguish the rotavirus enteritis with bacterial enteritis. The level of CK - MB can not only reflect the level of myocardial damage,but also judge the severity of rotavirus enteritis with the level of CRP.
