摘　　要：目的探讨IL-28B基因多态性对慢性丙型肝炎干扰素治疗应答的影响。方法 85例慢性丙型肝炎患者接受PEG-IFNa-2a联合利巴韦林抗病毒治疗48周,治疗结束后,随访24周。采用PCR产物直接测序法对85例患者的IL-28B rs12979860进行测序分型,分析rs12979860基因多态性与慢性HCV感染者PEG-IFNa-2a联合利巴韦林抗病毒疗效间的关系。结果 85例患者中IL-28Brs12979860位点CC型70例,CT型15例,未发现TT型;SVR获得率77.6%,rs12979860位点CC、CT两类基因型在SVR组及NSVR组中的分布差异具有统计学意义(χ~2=6.204,P=0.013);C、T等位基因在两组中分布差异亦具有统计学意义(χ~2=5.603,P=0.018)。结论 IL-28Brs12979860位点基因多态性对预测慢性丙型肝炎患者接受PEG-IFNa-2a联合利巴韦林抗病毒治疗的疗效具有一定的指导意义。Objective To identify the relationship between genetic polymorphisms in IL-28 Bgene and treatment of HCV infection.Methods A total of 85 chronic hepatitis C（CHC）patients were treated with peginterferon alfa-2 aplus ribavirin for 48 weeks,which were followed for 24 weeks after the end of treatment.The SNP rs12979860 of IL-28 Bwere analyzed.Results Of the 85 patients,two genotypes of IL-28B rs12979860 were found：CC were 70 cases and CT were 15 cases.66（77.6%）patients achieved sustained virologic response（SVR）.Rs12979860 genotype CC and CT were significantly different between SVR and non-SVR patients（χ2=6.204,P=0.013）.Rs12979860 Cand T alleles were also significantly different between SVR and non-SVR patients（χ2=5.603,P=0.018）.Conclusion IL-28B rs12979860 polymorphism is a better predictor for treatment of peginterferon alfa-2 aplus ribavirin in CHC patients with hepatitis C virus than other factors.
