摘　　要：目的探讨幕上原始神经外胚层肿瘤特征性MRI表现及其临床诊断价值。方法总结分析经临床证实的7例幕上原始神经外胚层肿瘤患者的临床及影像资料,并结合相关文献报道进行回顾性分析,所有病例均行MRI平扫及增强检查。结果 7例均为单发病灶,位置较浅,均累及灰、白质,病灶4例位于额颞叶,3例位于颞顶叶。1例瘤周未见明显水肿,5例瘤周可见轻度水肿,1例瘤周可见中度水肿。肿瘤实性部分在T1WI呈稍低信号或中等信号;T2WI呈中等信号或稍高信号。增强检查均明显强化。结论幕上原始神经外胚层肿瘤的MRI表现具有一定的特征性,对中枢神经细胞瘤的诊断具有重要价值。确诊仍需病理组织学及免疫组化检查。Objective To investigate the clinical manifestation and diagnostic value of MRI in the supertentorial primitive neuroectodemal tumor（ SPNET）. Methods A retrospective analysis was made of 7 clinically proven supertentorial primitive neuroectodemal tumor patients MRI（ Magnetic resonance imaging） data and combined with other reports. All cases were examined by plain and enhancement. Results All cases showed single intracranial mass,the location was superficial including gray and white matters. The tumors located respectively in the temporal-parietal lobes（ n = 4）,frontal-temporal lobes（ n = 3）. There was no peritumoral edema in1 case,slight edema in 5 cases and medium edema in 1 case. The solid portion of tumors showed slightly hypointense to isointense on T1-weighted images,and isointense to slightly hyperintense on T2-weighted images.All of lesions showed obvious enhancement. Conclusion MRI of the SPNET had characteristic appearances that had significant values for the diagnosis of the PNET,while diagnosis of the PNET still depends on the pathological histology and immuno-histochemical method.
