摘 要:目的了解停乳链球菌似马亚种临床分布特点及耐药性现状,为临床感染分析及指导合理用药提供依据。方法运用WHONET 5.6软件回顾性分析2013年1月-2016年12月杭州市中医院分离到的92株停乳链球菌似马亚种的临床致病特点及与耐药情况。结果 92株停乳链球菌似马亚种在痰液标本的分离率最高,为59.78%,其他依次为咽拭子、血液、创面脓液、宫颈拭子和尿液;临床分布以老年病科和儿科为主,分别占30.43%和15.22%。药敏结果显示92株菌的药敏表型较为一致,该菌对青霉素、阿莫西林、头孢噻肟、左旋氧氟沙星、氯霉素、万古霉素、复方新诺明均未出现耐药;对喹奴普丁-达副普汀、克林霉素耐药率较高且呈上升趋势;对红霉素耐药率最高,为100.00%。结论停乳链球菌似马亚种主要引起弱势人群的感染,临床应给予充分重视,并根据耐药性监测合理使用抗生素,减少广谱抗生素滥用情况。Objective To understand the clinical infection distribution characteristics and drug sensitivity of Streptococcus dysgalactiae subspecies equisimilis recently,so as to provide basis for the infection analysis and guidance for rational use of antibiotics in clinical. Methods Retrospectively,in compliance with the use of the WHONET 5. 6,92 strains of Streptococcus dysgalactiae subspecies equisimilis from the Traditional Chinese Medical Hospital of Hangzhou during January 2013-December 2016 were conducted for the analysis of clinical pathogenic characteristics and drug sensitivity. Results 92 strains Streptococcus dysgalactiae subspecies equisimilis in sputum samples were of the highest separation rate,accounting for 59. 78%,followed by throat swabs,blood,wound pus,cervical swabs and urine; which clinically mainly distributed in the Departments of Geriatrics and Pediatrics respectively,accounting for 30. 43% and 15. 22%. The results of antimicrobial susceptibility showed that the susceptibility phenotype of 92 strains was consistent. The bacteria had no resistance to penicillin,amoxicillin,cefotaxime,levofloxacin,chloromycetin,vancomycin and cotrimoxazole. The resistance rate of it to Quinupristin/dalfopristin,clindamycin showed an upward trend; and the highest was to erythromycin,accounting for 100. 00%. Conclusion Streptococcus dysgalactiae subspecies equisimilis mainly cause infection in vulnerable groups,clinical should pay full attention,and based on drug resistance surveillance,the rational use of antibiotics should be adopted to reduce the spread of broad-spectrum antibiotics.
