摘 要:目的探讨水疗联合熏蒸技术对四肢骨折术后肿胀及疼痛的影响。方法选取2015年1月~2017年1月徐州市中心医院收治的四肢骨折术后患者148例,按随机数字表法分为两组,对照组(74例)予以水疗治疗,研究组(74例)在对照组基础上予以中药熏蒸治疗,观察并记录两组间患肢肿胀程度、疼痛、临近关节活动度评分、血清骨代谢指标水平、骨痂生长评分,同时比较临床疗效。结果对照组总有效率低于研究组,差异有统计学意义(P<0.05);与治疗前比较,两组患者治疗后肿胀程度、疼痛VAS评分、临近关节活动度评分较低,差异有高度统计学意义(P<0.01),且研究组低于对照组,差异有高度统计学意义(P<0.01);两组患者治疗后血清碱性磷酸酶、骨形态发生蛋白-2、骨钙素及Ⅰ型前胶原羧基端肽水平较高,差异有高度统计学意义(P<0.01),且研究组高于对照组,差异有统计学意义(P<0.05或P<0.01);两组患者治疗后骨痂量、骨痂密度、骨痂边缘、断端边缘评分较高,差异有高度统计学意义(P<0.01),且研究组高于对照组,差异有高度统计学意义(P<0.01)。结论水疗联合熏蒸技术能明显减轻四肢骨折术后肿胀、疼痛及临近关节活动障碍,提高骨形成相关指标水平,促进骨痂生长及骨折愈合。Objective To investigate the effect of hydrotherapy combined with fumigation on swelling and pain of limb fracture after operation. Methods From January 2015 to January 2017, in Xuzhou Central Hospital, 148 patients with limb fracture after operation were selected and divided into two groups, control group(74 cases) was treated with hydrotherapy, experiment group(74 cases) was treated with herbal fumigation on the basis of the control group, the degree of swelling,pain, joint mobility score, serum bone metabolism index, callus growth score, and the clinical effect after the treatment were compared. Results The total effective rate in the control group was lower than that in the experiment group, the differences were statistically significant(P〈0.05); compared with before treatment, the degree of swelling, pain VAS score and near joint activity score lower in two groups after treatment were lower, the differences were statistically significant(P〈0.01), and that in the experiment group were lower than the control group, the differences were statistically significant(P〈0.01); serum levels of alkaline phosphatase, bone morphogenetic protein-2, osteocalcin and type Ⅰ procollagen carboxyl terminal peptide in two groups after treatment were higher, the differences were statistically significant(P〈0.01),and that in the experiment group were higher than the control group, the differences were statistically significant(P〈0.05 or P〈0.01); the amount of callus, callus density, callus edge, and the score of the broken edge in two groups after treatment were higher, the differences were statistically significant(P〈0.01), and that in the experiment group were higher than the control group, the differences were statistically significant(P〈0.01).
