经尿道膀胱逼尿肌间断横行等离子电切术治疗犬不稳定膀胱的尿动力学变化研究

摘  要:目的:建立"犬不稳定膀胱腔内手术切断部分逼尿肌"动物模型,使犬膀胱处于"低张、大容量"状态,通过对膀胱逼尿肌切断术后尿动力的研究,探讨经尿道微创治疗对改变逼尿肌不稳定收缩的可行性。方法:首先建立犬不稳定膀胱模型,解除梗阻3个月后在静脉麻醉下行经尿道膀胱逼尿肌间断横行等离子电切术。术前术后检测犬最大逼尿肌压力(MDP)、最大膀胱容量(MCC)、最大膀胱顺应性(MBC)等尿动力学参数。结果:7只不稳定膀胱模型犬,术后3个月后测最大逼尿肌压力(MDP)、最大膀胱容量(MCC)、最大膀胱顺应性(MBC)较术前有不同程度改善。结论:犬梗阻性不稳定膀胱经尿道膀胱逼尿肌横行等离子电切术能改善膀胱功能但仍需大样本支持。Objective:To establish the experimental canine unstable bladder models,test the changes in urodynamics after resecting the detrusor by endoscopic approach to achieve low pressure and large capacity state,to evaluate the feasibility of minimally invasive transurethral treatment for detrusor instability.Method:To begin with,set up the dog unstable bladder model.Next,remove bladder outlet obstruction.Finally,three months later under intravenous anesthesia transurethral plasma cut the bladder detrusor with discontinuous and transverse procedure.Maximum detrusor pressure(MDP),maximum cystometric capacity(MCC),and maximum bladder compliance(MBC)were tested preoperatively and postoperatively respectively.Result:Canine model of unstable bladder after surgery was built successfully in seven animals,and MDP,MCC and MBC significantly improved comparing to those before the surgery respectively.Conclusion:Unstable bladder detrusor plasma resection can improve bladder function in dogs due to bladder outlet obstruction,but larger samples are needed to be confirmed.
