摘  要:目的探讨术中超声引导下显微手术切除脑内型海绵状血管畸形的应用价值。方法回顾性分析15例脑内型海绵状血管畸形病例资料,使用术中超声定位和显影病变,辅助设计手术入路与监控切除程度。结果 15例脑内型海绵状血管畸形均全切除,术中超声定位病变准确,显影清晰。术后2例病人出现偏瘫加重,经康复治疗后肢体运动功能改善。13例病人无明显新发神经功能并发症。结论在手术切除脑内型海绵状血管畸形过程中,术中超声具有精准定位病变,正确指导设计手术入路和实时监控切除程度等特点,能有效保护正常脑组织,减少术后并发症。Objective To explore the clinical value of intraoperative ultrasound in performing micro-neurosurgical resection of cerebral cavernous malformation(CCM). Methods The clinical data of 15 CCM patients were analyzed retrospectively. During the operation,intraoperative ultrasound was used for localization, visualization, aided design of the operative approach and resection control of the lesions. Results Total resection was achieved in 15 patients and the lesions of all 15 patients were found accurately and satisfactorily displayed. Hemiplegia deterioration was observed in 2 patients whose movement function was improved through rehabilitation. No newly operation-related neurological complication was found in the remaining 13 cases. Conclusions Intraoperative ultrasound could serve as a useful tool to locate CCM accurately, guide the operative approach properly and control the extent of resection realtimely. It is suitable for minimally invasive surgery in resection of CCM under intraoperative ultrasound, which could effectively help to protect the normal brain tissue and reduce postoperative complications.
