腰麻术后患者自由卧位与舒适度关系的探讨

摘　　要：目的:研究蛛网膜下腔麻醉(腰麻)患者术后自由卧位与舒适度的关系。方法:216例行腰麻手术的患者随机分为两组各108例,对照组术后采取常规去枕平卧位6 h,观察组术后按以往习惯采取去枕平卧或侧卧、垫枕平卧或侧卧,翻身时要求保持头部与脊柱在水平位,可自由改变体位,不可坐起。结果:两组术后头痛、头晕发生率及生命体征情况比较,差异无统计学意义(均P>0.05);两组术后尿潴留比较差异有统计学意义(P<0.05);观察组舒适度评分显著高于对照组(P<0.05)。结论:腰麻术后患者若无特殊医嘱、生命体征平稳的情况下可采取自由卧位,有利于提高患者舒适度。Objective: To investigate the relationship between free decubitus and comfort of patients after subarachnoid anesthesia( lumbar anesthesia). Methods: 216 patients after lumbar anesthesia surgery were randomly divided into the control group and the observation group,with 108 cases in each group. The patients in the control group maintained supined without pillow routinely for 6 hours,while the patients in the observation group were assisted with free position,like with pillow in the supined decubitus or lateral,or without pillow in the supined decubitus or lateral; When turning over,the head and spine should be in a horizontal position; they can change the position freely but can not sit up. but can not sit up. Results: There were no significant differences in headache occurence and vital signs in both groups( P > 0. 05),while significant difference was observed in urinary retention and the degree of comfort between the two groups( P <0. 05). The score of the observation group was significantly higher than that of the control group on the degree of comfort( P < 0. 05). Conclusion: After surgery of lumbar anesthesia,free position is suggested to improve the comfort on the premise of no-specificness medical advice and stable vital signs.
