鼻内镜经泪前隐窝入路治疗真菌性上颌窦炎16例临床分析

摘　　要：目的:探讨鼻内镜泪前隐窝入路治疗真菌性上颌窦炎的疗效。方法:回顾性分析16例鼻内镜经泪前隐窝入路治疗真菌性上颌窦炎患者临床资料,其中复发性真菌性上颌窦炎2例,观察其临床效果。结果:所有患者术后病理均证实为真菌感染,未侵犯窦腔黏膜和骨质。所有患者随访12~24个月,均未复发,术腔黏膜恢复正常状态,下鼻甲形态良好,无溢泪发生。结论:经鼻内镜泪前隐窝入路治疗真菌性上颌窦炎创伤小,手术视野广阔清晰,可将病变处精确完整的切除,降低复发率,并且术后黏膜的生理结构和功能恢复良好,对于真菌性上颌窦炎是一个值得选择的术式。Objective： To evaluate the efficacy of endoscopic anterior lacrimal recess approach in the treatment of fungal maxillary sinusitis. Methods： The clinical data of 16 patients with fungal maxillary sinusitis were retrospectively analyzed,including recurrent fungal maxillary sinusitis（ n = 2）. Results： All the patients were confirmed to be fungal infection after operation. All the patients were followed up for 12 to 24 months,none of them recurred,the mucosa of the operation cavity recovered to normal state,and the inferior turbinate was in good shape,and no tears occurred. Conclusion： Nasal endoscopic anterior lacrimal fossa approach in the treatment of fungal maxillary sinusitis with small trauma surgery,broad vision can be clear,accurate and complete resection of the lesions,reduce the recurrence rate,and postoperative physiological structure and function of the mucous membrane of the good recovery for fungal maxillary sinusitis is a value choice procedure.
