Ⅳ型便携式睡眠呼吸监测仪对阻塞性睡眠呼吸暂停低通气综合征的诊断价值

摘　　要：目的探讨Ⅳ型便携式睡眠监测仪(PDMs)对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的诊断价值。方法通过门诊筛查打鼾患者102例,使用Ⅳ型PDMs和多导睡眠监测(PSG)同时对每位患者行睡眠监测,记录最低血氧饱和度(LSa O2)、睡眠呼吸紊乱指数(AHI),运用Clopper-pearson法计算诊断符合率,Bland-Altman图行一致性分析,评价其诊断价值。结果两者诊断一致率为97.05%,灵敏度为100.00%,特异度为76.92%,Kappa=0.853(P<0.001);两者对OSAHS分度一致率为82.35%,Kappa=0.742(P<0.001)。作Bland-Altman图分析,结果一致性非常高。结论Ⅳ型PDMs对OSAHS患者有较高诊断价值。Objective To investigate the diagnostic value of type IV portable sleep monitor （PDMs） in pa-tients with obstructive sleep apnea hypopnea syndrome （OSAHS）. Methods 102 patients with snoring were given type IV PDMs and polysomnography （PSG） for sleep monitoring. The lowest oxygen saturation （ LSa02） and sleep apnea hypopnea index （AHI） were recorded, and the diagnostic rate was calculated by Clopper-pearson method, and consistency by Bland-Altman chart analysis. Results The diagnostic concordance rate of the two groups was 97. 05% , the sensitivity 100% , the specificity 76. 92% and Kappa =0. 853 （ P 〈0. 001 ）. The concordance rate of OSAHS between the two groups was 82. 35% , and Kappa = 0. 742 （ P 〈 0. 001 ）. Bland-Altman diagram analysis showed high consistency. Conclusion PDMs has high diagnostic value in patients with OSAHS.
