孟鲁司特钠片联合布地奈德对咳嗽变异性哮喘患者免疫功能及支气管激发试验中肺功能的影响分析

摘　　要：目的探讨孟鲁司特钠联合布地奈德对咳嗽变异性哮喘患者免疫功能及支气管激发试验中肺功能及免疫功能的影响。方法将2013年-2015年于我院就诊的患者86例数字随机分组法分为两组,观察组与对照组各43例,其中对照组患者采用孟鲁司特钠进行治疗,观察组则采用孟鲁司特钠联合布地奈德进行治疗。两组治疗12周后,对比两组临床疗效、免疫功能指标(CD+8、CD+4以及CD+4)、支气管激发试验肺功能指标(FEV1一秒用力呼气容积、PEF峰值流速)的差异,并对不良反应发生情况及病情复发率进行记录。结果与对照组相比,观察组疗效显著(临床总有效率为93.0%),且差异具有统计学意义(P<0.05);治疗后,观察组患者CD+3[(65.77±4.48)vs(63.44±4.58)]以及CD+4[(43.06±5.67)vs(37.32±4.52)]水平明显高于对照组,CD+8[(23.87±5.02)vs(27.91±4.37)]水平明显低于对照组,且差异具有统计学意义(P<0.05);治疗后观察组患者FEV1.0[(2.51±0.44)vs(2.25±0.48)]、PEF[(69.93±6.38)vs(63.88±5.42)]值均明显高于对照组,且差异具有统计学意义(P<0.05)。治疗期间两组均无发生不良反应。观察期间观察组复发率(2.3%)明显低于对照组(16.3%),且差异具有统计学意义(P<0.05)。结论孟鲁司特钠片联合布地奈德能够显著改善咳嗽变异性哮喘患者的肺部功能,调节患者免疫功能,同时联合用药能够有效抑制炎症反应,提高治疗效果。Objective To investigate the effect of montelukast sodium tablets combined with budesonide on immune function and lung function of bronchial provocation test in patients with cough variant asthma. Methods 86 cough variant asthma patients from 2013 to 2015 were selected and randomly divided into two groups, 43 cases in each group. The montelukast sodium tablets oral treatment was used to the control group, while montelukast sodium tablets oral treatment combined with budesonide inhalation was given to the observation group. The clinical effect, immune function （ CD8＋ , CD4＋ and CD3＋ ） , lung function of bronchial provocation test （ FEV! forced expiratory vol-ume in one second, PEF peak expiratory flow） , side effects and recurrence were compared between the two groups af-ter 12 weeks. Results Compared with the control group, the clinical effect of the observation group （93% ） was sig-nificantly higher than that of the control group （ P 〈 0. 05 ） . The levels of CD3＋ and CD4＋ were much higher in the ob-servation groups after treatment were much higher than in the control group [ （ 65. 77 ± 4. 48 ） vs （ 63. 44 ± 4. 58 ） ; （43. 06 ±5. 67）仍（37. 32 ±4. 52） ],while the level of CDg＋ was lower in the observation group than in the control group [（23. 87 ± 5. 02） m （27. 91 ± 4. 37） ] （ all P 〈 0. 05 ） . The value of FEV：. 0 and PEF was obviously higher in the observation group than in the control group [（ 2. 51 ± 0. 44 ） m （ 2. 25 ± 0. 48 ） ; （ 69. 93 ± 6. 38 ） m （ 63. 88 ± 5. 42） ] （P 〈0. 05）. There was no adverse reactions occurred during the treatment. The relapse rate of the observa-tion group （2. 3% ） was lower than those of the control group （16. 3% ） （ P 〈 0. 05 ） .
