鼻内窥镜下手术联合双氧水冲洗治疗霉菌性上颌窦炎的疗效分析

摘　　要：目的探讨鼻内窥镜下手术联合双氧水冲洗治疗霉菌性上颌窦炎的临床疗效。方法随机从2015年1月—2017年3月该院收治的霉菌性上颌窦炎患者中抽取100例为研究对象,依治疗方式不同分为实验组(鼻内镜下上颌窦开放术+上颌窦霉菌取出术+双氧水冲洗,50例)和对照组(鼻内镜下上颌窦开放术+上颌窦霉菌取出术,50例)。统计两组治疗有效率、并发症发生率,术后随访,观察炎症复发情况。结果实验组治愈49例,有效1例,治愈率(98.0%)和总有效率(100.0%)均高于对照组(P<0.05);两组术后窦口开放良好,均无并发症,并发症发生率0.0%(P>0.05);实验组术后随访无复发病例(0.0%),对照组11例复发(22.0%),差异有统计学意义(P<0.05)。结论鼻内窥镜下手术联合双氧水冲洗治疗霉菌性上颌窦炎效果显著,可彻底清除病变,有效防止复发,而且治疗安全可靠,损伤小,无并发症,值得推广使用。Objective To study the curative effect of nasal endoscopic surgery and hydrogen peroxide washing in treatment of mycotic maxillary sinusitis. Methods 100 cases of patients with mycotic maxillary sinusitis admitted and treated in our hospital from January 2015 to March 2017 were randomly selected and divided into two groups according to the treatment method with 50 cases in each, the experimental group were treated with nasal endoscopic maxillary sinus opening and maxillary antrum mould removal and hydrogen peroxide washing, while the control group were treated with nasal endoscopic maxillary sinus opening and maxillary antrum mould removal, and the treatment effective rate and incidence rate of complications of the two groups were counted and the recurrence of inflammatory was observed. Results In the experimental group,there were 49 cases cured and 1 effective case, and the cure rate and total effective rate were respectively 98.0% and100.0%, which were higher than those in the control group(P<0.05), and the postoperative ostiums opertures opening was good and there were no complications and the incidence rate of complications was(0.0%)(P>0.05), and there was no recurrence case after surgery(0.0%), and in the control group, 11 cases were recurrent, accounting for 22.0%, and the difference was obvious(P<0.05). Conclusion The effect of nasal endoscopic surgery and hydrogen peroxide washing in treatment of mycotic maxillary sinusitis is obvious, which can completely clear the lesions and effectively prevent the recurrence and the treatment is safe and reliable with small injury and no complications, and it is worth promotion and application.
