高频彩色多普勒超声在浅表软组织肿物诊断及鉴别诊断中的应用

摘　　要：目的探讨高频彩色多普勒超声在浅表软组织肿物诊断及鉴别诊断中的应用。方法回顾性分析该院2013年1月—2016年3月127例浅表软组织肿物的声像图进行了分析。所有病例均经穿刺活检或手术病理证实,该文分析了肿物的部位、形态大小、内部回声、边界及其与周围组织的关系,以及病变周边与内部血流分布情况。结果超声对浅表软组织肿物的显示率为100.00%,诊断准确率为81.89%。其中115例为良性肿物,12例为恶性肿物,良性肿瘤超声表现为囊性或实性低回声结节,边界清楚,除血管瘤和部分神经源性肿瘤、炎性包块外有血流,其它基本上未见血流信号。恶性肿瘤肿块较大,边界欠清,且可检出较丰富血流信号。结论高频彩色多普勒超声具有高分辨率,可清晰显示肿物位置及与周围组织关系,推断其性质,并具有无创性、操作简便、经济等优点,在浅表软组织肿物诊断及鉴别诊断中具有重要的价值。特别是对肿物的穿刺活检及手术定位具有指导意义。Objective To study the application of high frequency color Doppler ultrasound in diagnosis and differential diagnosis of superficial soft tissue mass. Methods The ultrasonograms of 127 cases with superficial soft tissue mass in our hospital from January 2013 to March 2016 were selected and all patients were confirmed by the needle biopsy or operative pathology, and the mass size, shape size, internal echo, boundary and correlation with the surrounding tissues, lesions and internal blood flow distribution situation were analyzed. Results The display rate of ultrasound to the superficial soft tissue mass and diagnosis accurate rate were respectively 100.00% and 81.89%, including 115 cases of benign masses and 13 cases of malignant masses, and the benign tumor ultrasound manifestations were cystic or solid hypoechoic nodules, and the boundary was clear, and the other had no blood flow signals except for angioneoplasm and inflammatory masses. Conclusion The resolution rate of high frequency color Doppler ultrasound is high, which can clearly show the mass site and correlation with the surrounding tissues with non-trauma, simple operation and economic advantages and it is of important value in the diagnosis and differential diagnosis of superficial soft tissue mass, especially it is of guidance significance to the needle biopsy and operative positioning of masses.
