高血压病患者ST段缺血样改变常规心电图与动态心电图对比分析

摘　　要：目的对比分析高血压患者ST段缺血样改变常规心电图(ECG)与动态心电图(DCG)表现。方法本次研究选择了在2015年6月至2016年6月阶段在我院进行治疗的原发性高血压患者100例,100例患者分别行常规心电图检查和动态心电图检查。结果动态心电图ST段缺血样改变阳性率明显高于常规心电图,二者比较差异显著。随着高血压分级的升高,动态心电图和常规心电图检查对患者的ST段缺血样改变检出率也随之升高,差异均显著;但动态心电图对各个高血压分级的ST段缺血样改变检出率均显著高于常规心电图。16例常规心电图确诊为ST段缺血样改变的患者,进行动态心电图监测时发现ST段下移幅度>常规心电图显示的结果,且呈持续性改变。结论与常规心电图检查相比,动态心电图检查是目前判断原发性高血压患者早期ST段缺血样改变比较敏感又安全的方法。Objective To investigate the features of ischemic ST-segment changes on routine versus ambulatory electrocardiography in patients with hypertension. Methods A total of 100 patients with primary hypertension who were treated in our hospital from June 2015 to June 2016 were enrolled and underwent routine or ambulatory electrocardiography. Results Ambulatory electrocardiography had a significantly higher positive rate of ischemic ST- segment changes than routine electrocardiography. The detection rate of ischemic ST-segment changes by ambulatory and routine electrocardiography increased significantly with the increase in hypertension grade, but compared with routine electrocardiography, ambulatory electrocardiography had a significantly higher detection rate of ischemic ST-segment changes in patients with different hypertension grades. Ambulatory electrocardiography performed for 16 patients with ischemic ST-segment changes confirmed by routine electrocardiography showed greater ST-segment depression than routine electrocardiography and presented persistent changes. Conclusion Compared with routine electrocardiography, ambulatory electrocardiography is more sensitive and safer in identifying early-stage ischemic ST-segment changes in patients with primary hypertension.
