摘  要:目的分析比较MR扩散加权成像(DWI)和CT灌注成像(CT-PI)对肝纤维化的诊断价值。方法选取2013年5月至2015年5月收治的经活检证实肝纤维化患者60例作为肝纤维化组,另择同期健康体检人群60名作为对照组,两组均采用DWI检查和CT-PI检查,比较两组CT灌注参数及诊断肝纤维化的结果。结果肝纤维化组肝动脉灌注分数(HAF)高于对照组,差异有统计学意义(P<0.05)。DWI经过ROC曲线统计,其AZ值为0.88,位于0.7~0.9,诊断价值中等,其敏感度为80.00%,特异度为93.33%;CT-PI的AZ值为0.69,位于0.5~0.7,诊断价值较低,其敏感度为61.67%,特异度为70.00%。DWI诊断准确度、敏感度和特异度明显高于CT-PI,差异均有统计学意义(P<0.05)。结论与CT-PI对比,MR-DWI在诊断肝纤维化方面具有更为可靠的诊断结果,其诊断价值更高。ObjectiveTo explore the diagnostic value of DWI and CT perfusion in diagnosis of liver fibrosis. MethodsFrom May 2013 to May 2015, 60 hepatic fibrosis patients and 60 healthy people using DWI and CT perfusion imaging was performed in two subjects, liver CT the perfusion parameters of fibrosis group and normal control group, comparison of two kinds of examination methods for the diagnosis of hepatic fibrosis results. ResultsCompared with the control group, HAF was significantly higher in the hepatic fibrosis group, and the difference between the two groups was statistically significant(P〈0.05). DWI by ROC curve, the AZ value is 0.88, is located in 0.7 to 0.9, the diagnostic value of medium, the sensitivity was 80%, specificity was 93.33%.While the AZ value of CT-PI is 0.69, is located in 0.5 to 0.7, the diagnostic value is low, the sensitivity was 61.67%, specificity was 70%. The accuracy, sensitivity and specificity of DWI were significantly higher than those of CT-PI, and the difference was significant(P〈0.05). ConclusionCompared with CT-PI, DWI has more reliable diagnostic results in the diagnosis of hepatic fibrosis, and its diagnostic value is higher.
