摘 要:目的比较128层螺旋CT主动脉成像中3处触发检测点对对比剂剂量的影响。以期获得多层螺旋CT主动脉成像合理的触发检测点及对比剂剂量。方法通过排列区分组法随机将156例患者分为3组,各52例。由2名放射科医师盲法评价不同触发检测点对成像的影响。成像中对比剂为碘佛醇(350 mg I/ml),注射速度为4.0 ml/s。第1组触发检测点为主动脉升部,对比剂剂量为70 ml;第2组触发检测点为主动脉弓降部,对比剂剂量为65 ml;第3组触发检测点为肺动脉主干,对比剂剂量为55 ml。测定主动脉强化后平均CT值,统计分析3组触发检测点下主动脉强化后平均CT值及图像质量。结果第1组中10例触发失败,42例触发成功;第2组中52例全部触发成功;第3组中52例全部触发成功。第1组、第2组和第3组触发成功时主动脉强化后平均CT值分别为(275.5±12.5)HU,(301.3±15.8)HU,(293.5±21.5)HU。3组图像质量:第1组48例为优,4例为良;第2组和第3组均为优。3组均满足诊断准确率100.0%,差异无统计学意义(P>0.05)。结论以肺动脉主干为触发检测点行128层螺旋CT主动脉成像可行且对比剂使用剂量小,优越性明显。ObjectiveTo investigate the impact of different triger monitoring points on the contrast media doses in 128-slice spiral CT aortic angiography. Methods156 patients who underwent aortic CT angiography were randomly divided into 3 groups. There were 52 patients in each group. Two doctors evaluated blindedly the effect of monitoring site on the image quality. Ioversol (350 mgI/ml) was used as contrast media, and the injection speed was 4.0 ml/s. In the first group, the triger monitoring point was ascending part of aortic arch, and the contrast media dose was 70 ml. In the second group, the triger monitoring point was the descending part of aortic arch, and the contrast media dose was 65 ml. In the third group, the triger monitoring point was the main pulmonary artery, and the contrast media dose was 55 ml. The mean CT values of aorta enhancement were measured. The mean CT values and image quality of three groups were comparatively analyzed. ResultsI0 patients were failed to be triggered in the first group, while in the second and third group, all the patients were triggered successfully. Moreover, the mean CT value of aorta enhancement in the first, second and third group were (275.5±12.5) HU, (301.3±15.8) HU and (293.5±21.5) HU, respectively. The image quality results showed that the images of 48 patients were excellent and those of 4 patients were good in the first group, and the images were all excellent in the second and third group. The rate of satisfaction was 100% in each group (P〉0.05). The contrast media doses in the first, second and third group were 70, 65, 55 ml,respectively. Therefore, in the third group, the contrast media dose were the least. ConclusionTaking the main pulmonary artery as the triger monitoring point in 128-slice spiral CT aortic angiography is feasible, which is able to decrease contrast media dose with obvious superiorities. It is worth to clinical promotion.
