子宫瘢痕憩室MR动态增强与经阴道彩色多普勒超声的临床价值分析

摘　　要：目的:比较经阴道彩色多普勒超声、磁共振动态增强对剖宫产子宫瘢痕憩室(CSD)的临床诊断价值。方法:回顾性分析经宫腔镜证实的32例同时做过阴道超声与MR的CSD病例,对其声像图及CDFI特点与磁共振动态增强影像学表现,对照手术结果进行回顾性分析。结果:经阴道超声与MR动态增强在CSD诊断中,对发现病变的敏感性基本一致,分别为93.75%和100%,对病灶的诊断准确率MR高于经阴道超声,分别为87.5%和96.87%。结论:经阴道超声检查具有操作简单、费用低、重复性好、无并发症等优势,是诊断CSD的首选方法;MR动态增强其空间分辨力及软组织分辨力高,能直观地了解憩室的形态,对子宫周围组织结构的进行全面的判断分析,不存在检查的"时间窗"问题,可为CSD的诊治提供直观详细的影像学依据。Purpose： To compare the value of transvaginal color Doppler ultrasound and dynamic contrastenhanced MRI in diagnosis of cesarean scar diverticulum （CSD）. Methods： A retrospective analysis was done in 32 patients with CSD confirmed by hysteroscopy. They were all undergone with transvaginal color Doppler ultrasound and dynamic contrast-enhanced MRI imaging, the CDFI characteristics and features of dynamic contrast-enhanced magnetic resonance imaging were analyzed. Results： The sensitivity of transvaginal ultrasound and dynamic contrast-enhanced MRI imaging in diagnosis of CSD was 93.75% and 100%, respectively. The accuracy rate of MR （96.87%） was higher than that of taransvaginal ultrasound （87.5%）. Conclusion： Transvaginal color Doppler ultrasound has the following advantages： simple, low cost, good repeatability, no complications and so on, it is the preferred method for diagnosis of CSD. Dynamic contrast-enhanced MRI has the characteristics of high spatial resolution and high resolution of soft tissue, it could be used to intuitively understand the overall shape of the diverticulum, and also conduct a comprehensive analysis to determine the structure of the uterine tissue surrounding,＂Time windows＂ problem of MR dynamic enhancement does not exist. It provides an intuitive and detailed imaging evidence for the diagnosis and treatment of CSD.
