小剂量新斯的明联用VitB1穴位注射治疗肛肠疾病术后尿潴留的应用价值

摘　　要：目的观察评价小剂量新斯的明联用VitB．穴位注射治疗肛肠疾病术后尿潴留（UR）的临床效果。方法选择2015年8月至2017年2月期间佛山市禅城区朝阳医院及佛山市中医院普外科收治的肛肠疾病术后尿潴留患者98例，随机对照试验分为两组，对照组49例给予新斯的明穴位注射治疗并采取常规护理方法；观察组49例给予小剂量新斯的明联用VitB，穴位注射治疗并采取优质综合护理干预，并比较两组的临床效果及护理满意度。结果观察组总有效率为97．96％，明显高于对照组的79．59％（P〈0．05），观察组膀胱残存尿量明显低于对照组（P〈0．05），观察组自行排尿时间、术后进食时间、排便时间和住院时间均显著短于对照组（均P〈0．05），观察组总满意度为100．00％，明显高于对照组的83．67％（P〈0．05）。结论小剂量新斯的明联用VitB．穴位注射治疗肛肠疾病术后尿潴留疗效显著，有效地降低了患者的膀胱残留尿量，大大缩短了自行排尿时间、术后进食时间、排便时间和住院时间，显著提高了护理满意度。Objective To evaluate the clinical effect of a low dose of neostigmine combined with VitBl acupoint injection for postoperative urinary retention （UR） of anorectal disease. Methods From August, 2015 to February, 2017, 98 patients with postoperative urinary retention treated at Chaoyang Hospital, Chancheng, Foshan and Foshan Hospital of Traditional Chinese Medicine were selected and divided into a control group and an observation group with randomized controlled trial （RCT）, 49 cases for each group. The control group were treated with neostigmine acupoint injection and were routinely cared. The observation group were treated with a low dose of neostigmine combined with VitBl acupoint injection and high quality comprehetlsive nursing intervention. The clinical effects and nursing satisfaction were compared between these two groups. Results The total effective rate was 97.96% in the observation group and was 79.59% in the control group （ P 〈0.05）. The residual urine volume was significantly lower, the total satisfaction rate was significantly higher, and the self-urination time, postoperative feeding time, defecation time, and hospitalization time were shorter in the observation group than in the control group （P 〈0.05）. The total satisfaction rate was 100.00% in the observation group and was 83.67% in the control group （P〈0.05）. Conclusions A small dose of neostigmine combined with VitBl injection in the treatment of postoperative urinary retention （UR） of anorectal disease is effective and can reduce the urinary volume of urinary bladder, greatly shorten self-urination time, postoperative feeding time, defecation time, and hospitalization time, and significantly increase nursing satisfaction.
