摘 要:目的分析四川口岸首例输入性三日疟感染病例的临床表现及实验室检测结果,为四川地区三日疟的预防控制提供参考依据。方法对2017年四川口岸1例疟疾患者采集血样,采用BinaxNow疟疾快速诊断试剂盒、瑞吉染色镜检和实时荧光PCR方法进行鉴定。结果疟疾快速诊断试剂盒检测患者血样为除恶性疟原虫以外的间日疟原虫、三日疟原虫或卵形疟原虫感染;血涂片镜检为三日疟原虫,密度为2 400/μl;实时荧光PCR检测三日疟原虫核酸阳性。结论结合血涂片、PCR等检测结果、流行病学资料和临床表现,该患者确诊为输入性三日疟原虫感染病例。提示应加强疟防专业人员三日疟诊治技能培训,以减少误诊误治。Objectives To investigate and analyze the clinical symptoms of and laboratory results from the first case of imported Plasmodium malariae at Sichuan Port and to provide a basis for further control efforts. Methods A patient with malaria was identified at Sichuan Port in 2017.Blood samples were collected and tested with the BinaxNowTM Malaria Test,Giemsa staining,and RT-PCR. Results The BinaxNowTM Malaria Test revealed that patient was infected with Plasmodium vivax,P.malariae,or P.ovale other than P.falciparum.Microscopy revealed P.malariae with a density of 2,400 per microliter.Real-time PCR yielded amplification products specific to P.malariae. Conclusion Based on blood smears,PCR results,epidemiological information,and clinical symptoms,the patient was definitively diagnosed with imported P.malariae malaria.Training of clinicians and laboratory staff should be enhanced in order to avoid misdiagnosis.Health education of migrant workers should be conducted so that malaria is promptly diagnosed and treated.
