护理路径在消化道早癌患者内镜下黏膜剥离术后的应用效果

摘　　要：目的探讨护理路径在消化道早癌患者内镜下黏膜剥离术后的应用效果。方法选取2015年6月至2016年10月间江苏省中医院收治的89例消化道早癌患者,采用随机数表法分为观察组和对照组,观察组45例,对照组44例。观察组患者在常规护理基础上采用护理路径进行护理干预,对照组患者采用常规护理。比较两组患者手术时间、在内镜中心时间、治疗情况、术后并发症发生情况及对护理工作的满意度。结果观察组患者术后并发症为创面出血0例,感染1例,腹痛3例,总发生率为8.9%;对照组患者为创面出血2例,感染3例,腹痛4例,总发生率为20.5%,两组比较,差异有统计学意义(P<0.05)。观察组患者手术时间及在内镜中心时间分别为(39.5±5.0)min和(115.0±15.0)min,明显低于对照组的(47.5±5.5)min和(136.0±19.0)min,两组比较,差异均有统计学意义(均P<0.01)。观察组患者护理总满意率97.8%,明显高于对照组患者的86.4%,两组比较,差异有统计学意义(P<0.01)。结论护理路径可以有效缩短内镜下黏膜剥离术治疗消化道早癌患者的手术时间及在内镜中心时间,减少术后并发症,提高护理满意度。Objective To investigate the application of nursing pathway in patients with early digestive tract cancer after endoscopic submucosal dissection.Methods Eighty-nine patients with early digestive tract cancer treated at Jiangsu Provincial Hospital of Traditional Chinese Medical were selected from June 2015 to October 2016.Patients were randomly divided into an observation group and a control group with 44 patients in each group.The observation group received nursing guided by the nursing pathway and the control group received routine nursing care.Operative time,length of stay at Center of Digestive Endoscopy,treatment situation,postoperative complications and satisfaction with nursing work were compared between the two groups.Results For the postoperative complications,no one was found to have wound bleeding,infection occurred in 1 patient,abdominal pain occurred in 3 patients and the overall incidence of complications was 8.9% in the observation group which were lower than in the control group（wound hemorrhage 2 patients,infection 3 patients,abdominal pain 4 patients,the overall incidence of complications20.5%）（P0.05）.Operative time and length of stay at Center of Digestive Endoscopy was（39.5±5.0） min and（115.0±15.0） min,respectively in the observation group which was significantly lower than（47.5±5.5） min and（136.0±19.0） min,respectively of the control group（all P0.05）.Four patients had a little wound bleeding after operation and bleeding was stopped by electric coagulation.The overall satisfaction rate was 97.8% in the observation group which was significantly higher than 86.4% of the control group（P0.01）.Conclusion Nursing pathway could effectively shorten the operative time and discharge time,reduce complications and improve satisfaction with nursing in patients with early digestive tract cancer after endoscopic submucosal dissection.
