摘 要:目的:探讨辐照生物敷料联合普鲁卡因在非复杂性烧伤创面中的应用效果及相关影响因素。方法:纳入2015年4月至2016年9月间台山市人民医院烧伤整形科收入的160例非复杂性烧伤创面患者,采用方便抽样法将其分为4组,分别为聚维酮碘凡纱覆盖创面治疗组(A1组)、聚维酮碘凡纱联合普鲁卡因治疗组(A2组)、辐照生物敷料治疗组(B1组)和辐照生物敷料联合普鲁卡因治疗组(B2组),每组40例,比较每组的创面愈合时间,探寻愈合不良的危险因素。结果:B2组愈合时间最短为(9.2±2.8)d,其次为B1(9.3±2.6)d、A2(10.4±3.6)d和A1(10.9±3.4)d,4组比较差异具有统计学意义(F=2.850,P<0.05),A1组愈合时间显著长于B2组(t=2.441,P=0.017);B2组有5例发生疼痛,A2组有7例,A1和B1组分别有32和28例,4组比较不良反应发生率差异具有统计学意义(χ~2=59.192,P<0.001),A1和B1组的疼痛发生例数显著多于B2组(χ~2=36.656和27.286,P<0.01);年龄≥41岁(OR=1.558,95%CI:0.875~2.414)以及合并糖尿病(OR=1.646,95%CI:0.965~4.210)是影响疗效的危险因素。结论:辐照生物敷料联合普鲁卡因对于非复杂性烧伤创面的治疗是有效、安全地,而且可缩短创面愈合时间、减轻患者疼痛感,值得临床推广应用。Objective: To explore the values of irradiated biological dressing (IBD) combined procaine in uncomplicated burn wounds. Method: 160 patients with uncomplicated burn wounds were recruited during April 2015 to September 2016 and then divided into povidone-iodine gauze group (A1, n=40), povidone-iodine gauze combined procaine group (A2, n=40), IBD group (B1, n=40) and IBD combined procaine group (B2, n=40). The wound healing time of each group was compared, and risk factors associated with adverse reactions were analyzed. Results: Healing time in B2 was (9.3±2.6) days, and B1, A2, A1 was (9.3±2.6) days, (10.4±3.6) days and (10.9±3.4) days respectively with significantly difference (F=2.850, P=0.039). The healing time of A 1 was significantly longer than that of B2 (t=2.441, P=0.017). There were 5 cases experiencing pain in B2, while 7 in A2, 32 in A1 and 28 in B1 with significantly difference(x2=59.192, P〈0.001). The incidence of pain in group A 1 and group B 1 was significantly higher than that in group B2 (,y 2=36.656 and 27.286, P〈0.001); above 41 years old (OR=1.558, 95%CI: 0.875-2.414) and complicated with diabetes (OR= 1.646, 95%CI: 0.965-4.210) were risk factors of the efficacy. Conclusion: Irradiated biological dressing combined procaine is effective and safe in the treatment of uncomplicated burn wounds, which could shorten healing time and relieve the pain of patients, worthy of clinical popularization and Application.
