腹腔镜卵巢囊肿剔除术治疗中电凝法与缝合法对临床疗效的影响分析

摘　　要：目的比较研究腹腔镜卵巢囊肿剔除术治疗中电凝法与缝合法对临床疗效及性激素水平的影响分析。方法选取2012年4月至2016年9月就诊的共148例卵巢囊肿患者作为研究对象。按随机数字表法将所有患者分为缝合组（n=72例）和电凝组（n=76例），缝合组患者进行腹腔镜卵巢囊肿手术中采用缝合法，而电凝组患者进行腹腔镜卵巢手术中采用电凝法。比较两组患者的术中出血量和手术时间、性激素水平和卵巢早衰发生率。结果缝合组术中出血量和手术时间明显高于电凝纽，且差异有统计学意义（P〈0．05）。两纽患者治疗前LH、FSH、E2水平比较无显著性差异（P〉0．05），两组患者术后1个月、3个月时LH、FSH、E2水平比较无显著性差异（P〉0．05），电凝组患者术后6个月时LH、FSH水平明显高于缝合组，E2水平明显低于缝合组，且差异均有统计学意义（P〈0．05）。电凝组患者卵巢早衰发生率高于缝合组，但差异无显著性（P〉0．05）。结论卵巢囊肿治疗中腹腔镜手术应用广泛，术中电凝止血会损伤卵巢功能，腹腔镜下卵巢缝合止血可减少卵巢损伤，保护卵巢功能，具有临床推广价值。Objective Compare influence of the clinical efficacy and hormone level of electrocoagulation and stitching in laparoscopic ovarian cystectomy. Methods 148 ovarian cyst cases in our hospital between April 2012 and March 2016 were selected and divided into two groups by digital table. The two groups were called stitching group （ n=72cases ） and electrocoagulation group （ n=76 cases ） , the patients of stitching group were treated by stitch method, while the patients of electrocoagulation group were treated by electrocoagulation. The amount of bleeding, time of operation, hormone level and rate of premature ovarian failure were compared. Results The amount of bleeding, time of operation of stitching group were much higher than electrocoagulation group （ P〈0.05 ） . The LH, FSH and E2 level of two groups before operation had no statistical difference （ P〉0.05 ） , the LH, FSH and E2 levels of two groups 1 month and 3 months after operation had no statistical difference （ P〉0.05 ） , the LH, FSH levels of electrocoagulation group 6 months after operation were much higher than those of stitching group, E2 was much less than stitching group （ P〈0.05 ） . The rate of premature ovarian failure of electrocoagulation group was higher than stitching group, but had no statistical difference （ P〉0.05 ） . Conclusion In the treatment of ovarian cyst, laparoscopic surgery is widely used, intraoperative electrocoagulation will damage the function of the ovary, laparoscopic ovarian stitching hemostasis can reduce ovarian damage, protect ovarian function, which is worthy ofclinical promotion.
